Provision of Personal Information. 54.1. A staff member may wish to access leave or other flexible working arrangements in order to accommodate their personal, family and xxxxx’s circumstances in accordance with the procedures provided in this Agreement. The staff member must disclose sufficient information relevant to these circumstances to enable the University to determine whether there are good and sufficient reasons for the University to approve access to leave and flexible work arrangements.
Provision of Personal Information. H.I.S. will provide the name, gender, age, address, telephone number, email address, or passport No. to transportation and accommodation facilities, insurance companies, or souvenir shops within the scope necessary for arrangement and receipt of travel services applied for by the customer, for insurance procedures to insure H.I.S. against liability under the Travel Contract and expenses at the occurrence of an accident, and for convenience of shopping by the customer at souvenir shops at the destination of the travel, via transmission by electronic means, etc. When the customer applies for travel, H.I.S. shall receive the consent of the customer to the provision of personal data. H.I.S. will not provide personal information to a third party without the approval of the customer except for in the following cases:

Customer will not provide Rocket Software with personal data unless the parties expressly agree in writing to the provision by Customer, and receipt by Rocket Software, of such data. Where the parties expressly agree in writing that personal data will be provided by Customer (“Customer Personal Data”) (i) Customer authorizes Rocket Software to process Customer Personal Data to the extent required to provide the Services and comply with its obligations at law (the “Permitted Uses”) and (ii) where applicable, the provisions of Section 10.2 shall apply.

4) I understand that Waseda University may use the personal information I have submitted in order to contact me and to invite me to events and various briefing sessions held by Waseda University as someone who has experienced studying abroad.
5) (Only for Programs by Traveling) Regarding the personal details of myself and my guardian which were provided at the time of purchase of the said insurance plan, registration at the crisis management service and arrival at the travel destination as well as details of any accidents I get involved in during my study abroad period (hereafter “personal information”) , I agree to share such information with: the Center for International Education, my School Office, Campus Insurance Center Co. Ltd., the insurance company designated by Waseda University, the crisis management agency, travel agencies, airline companies, and the relevant ministries and embassies to deal with accidents, contact me and my guardian, and administer the study abroad program.

5) I agree to have a copy of my passport with my personal information (such as my name, birth date, gender, passport number, and expiration date of the passport) to be shared with the travel agency and airline company by Waseda University if it becomes necessary for the university to make a tentative reservation for a return flight if I apply for a program that provides group flight airline tickets.
6) For the purpose of administering the study abroad program and responding to any emergencies, I agree to the sharing of the personal details of myself, my guardian, and risk management information including accidents during the study abroad period (hereafter “personal information”), which I have provided to Waseda University, the insurance company designated by Waseda University, the crisis management agency, travel agencies, airline companies, and host university through various procedure necessary for my participation in the study abroad program and the information will be shared, and that will be used by Waseda University, the insurance company designated by Waseda University, the crisis management agency, travel agencies, airline companies, and the relevant ministries and embassies.
Provision of Personal Information. 8.2.1 As a condition to using some aspects of the Services, you may be required to register with mySGInsider and select a password and username (“mySGInsider User ID”). If you are accessing the Services through a Third Party Website or service, mySGInsider may require that your mySGInsider User ID be the same as your username for such Third Party Website or service.
