Purchase of Service. The Department agrees to purchase and the Contractor agrees to furnish, on behalf of the Department for such children and families as the Department names in writing the following “Peer Support Specialists, Recovery Coaches, or Peer Mentor Services”:

Purchase of Service a one-time allotment for no more than $250 per child. This allotment is approved by ADSD to pay for a specific item or items, and is intended for emergency situations or for the purpose of purchasing needed medical/care supplies. Room and Board expenses include rent, utilities, phone, food costs, and other expenses related to housing. Service Coordination/Targeted Case Management (TCM) – service delivery consisting of assessment, referral, planning, linkage and monitoring provided by a service coordinator who is qualified by educational background and training to assist, advise, direct and oversee services to eligible children. Support services are those services identified in the assessment and planning process to address specific goals, mitigate health and safety issues and facilitate skill development that enables a child to live successfully in the home and community Supported Living Arrangement - supports provided to children usually with roommates, by staff of contracted provider agencies or individuals to develop and maintain skills needed to live independently in the home and community. Qualified providers are providers that have completed the agency certification process successfully and have been determined to have appropriately skilled staff to provide services to a specific child. Transportation expenses include bus passes, stipends for gas or other items related to traveling from one place to another place.

Regional Centers (RC) – Nonprofit private corporations that contract with the Department of Developmental Services to provide or coordinate services and supports for individuals with developmental disabilities. They have offices throughout California to provide a local resource to help find and access the many services available to individuals and their families. Regional centers provide diagnosis and assessment of eligibility and help plan, access, coordinate and monitor the services and supports that are needed because of a developmental disability. Regional Centers provide people with residential, day, transportation, and social, independent living, and respite, medical, psychological, preschool and other services.

Purchase of Service. Subject to Section of the Pensions (Municipal) Act the Employer agrees to participate as to (one-half) the cost determined by the Commissioner of Municipal Superannuation to extend the pensionable service of an employee covered by this Agreement up to a maximum of year. It is understood that this extension shall represent that period of time served by the employee in a probationary capacity as an employee of the Department and which has not heretofore been considered as pensionable service. This benefit shall be subject to the following conditions: only an employee with a vested interest in the Municipal Superannuation Plan and who has reached the minimum age of retirement as defined in the Pensions (Municipal) Act shall be eligible. Effective the date of ratification of the Memorandum of Agreement, employees who are not eligible for the benefit described in above, may make arrangements prior to April to purchase the full amount associated with the buy-back of service and, upon the employee producing the receipt, the Employer agrees to reimburse the employee percent (50%) of the purchase cost as stipulated by the Pension Corporation. This payment will be made in the year the employee reaches minimum retirement age.
Purchase of Service. Subject to terms and conditions set forth in this Contract, including the appendices outlined in Article V Section M below, which are on file at FCDJFS and available for review by the Contractor, the Contractor agrees to furnish those specific services detailed in this Contract.
