AO Application for All Entities Make application to, and if approved, enter into this agreement with the Department. Maintain at least one permanent, regularly occupied structure in Texas with a permanent Texas physical address. Own or lease, in the name of the driver training school or school owner, at least one Class A or B (Group) CMV that requires a CDL and is specific to the type of skills test being administered. Regional Education Service Centers that do not own or lease at least one Class A or B (Group) CMV and that are serving as a AO for a School District must present to the Department an agreement between the Regional Education Service Center and the School District certifying that the School District owns or leases at least one Class A or B (Group) CMV and the Regional Education Service Center has authorization to use the vehicle(s) for skills testing. Vehicles used for skills testing must have a current registration and safety inspection. Conduct at least ten complete CDL skills tests per calendar year, with the calendar year beginning every January 1. A complete CDL skills test includes the pre-trip, basic skills control, and road trip. For those Examiners who completed their CCE course training during the latter part of the year and are unable to conduct ten complete CDL skills tests in that calendar year, the Department considers this requirement met since a State examiner observed the successful administration of at least one skills test during the CCE course training. Starting the following calendar year, the Examiner must conduct the ten complete CDL skills tests.

The following are minimum requirements for a successful consultant for the position: - A university degree in any social science with specialized certification in linguistics, English or similar disciplines. - At least five years of experience in translation; - Ability and capacity to deliver translations under tight deadlines, especially for media release; - Work experience with regional or international organizations in the region; - Excellent English oral and written communication skills; - Native speaker of Khmer with excellent knowledge of the writing systems of the language; - Familiarity with natural resources management/environmental awareness is desirable; and - The translator shall adhere to the terms of reference, including the time schedule. The contractors shall understand that the time schedule provided in the ToR is tentative and that the contractors remain flexible within the period indicated below. Interested individuals are required to submit proposals consisting of two parts: - Technical proposal (cover letter, CV, portfolio/work samples, a list of clients, contact information of three references); and - Financial proposal (cost per word, or cost per page of average 300 words, in English as a source document). The financial proposal should be prepared in the following format, if translation fees are different by the type of documents (i.e. technical, simple public information, presentation slides, and tables). d. You agree that you will not intentionally provide false information when accessing or using the Payment Service.

Qualifications and Requirements. 1.2.1. CPs shall meet the following Qualifications.
1.2.1.1. The MRI Technologist shall have as a minimum a high school diploma. All MRI Technologists shall have completed an instructional program that follows the American Society of Radiologic Technologists (ASRT) curriculum guide and competencies based on skills identified by the American Registry of Radiologic Technologists (ARRT) and the Joint Review Committee on Education of Radiologic Technologists (JCERT).
1.2.1.2. The MRI Technologist shall possess current registration by the ARRT and current certification in the Magnetic Resonance Imaging modality.
1.2.1.3. The MRI technologist shall be active in the provision of MRI services and have provided MRI services for a minimum of three years within the past five years. Of the three years, the technologist shall have a minimum of 12 months experience under the supervision of a board certified or board eligible Radiologist. Additionally, the technologist performing the MRI exam shall have been trained on the type of unit the Contractor intends to place at WACH, for a period of not less than six months. The MRI technologist should be able to perform all exams but not limited to those listed on Attachment 5 under Current Procedural Terminology (CPT) Description.
1.2.1.4. The CPs must have and maintain current Basic Life Support (BLS) certification. The American Heart Association Basic Life Support for Healthcare Providers is the only program accepted. BLS recertification shall be maintained in a current status at all times while the CP is performing services under this contract.
