An employee wishing to take Voluntary Time Off with out pay (defined as hourly rate) may submit a request for a specific number of hours/days he/she wishes to take as VTO, on the VTO Request Form. The use of VTO is voluntary by the employee and can be withdrawn by the employee at any time. Agreement by the department head to a VTO schedule is voluntary and can be withdrawn by the department at any time. b. Joint agreement between the employee and his/her department head or designee is required and shall specify the exact hours/days to be taken off under VTO.

Request Submission. Each Request shall contain such information, such as the routes, the location of splice points, network architecture and diversity requirements, and the proposed commencement date, as Licensor may reasonably request to assist it in determining whether it will construct and/or license the capacity of Additional Facilities. If Licensor agrees to fulfill any Request, it shall deliver to Licensee a Cost Estimate Schedule, in the form of Exhibit A, setting forth its good faith estimate of the Allocated Cost (as defined on Annex A) of constructing such Additional Facilities.

Request Submission. As promptly as practicable following such notice, but in any case within thirty (30) days after such notice unless otherwise agreed by Owners, Contractor shall provide Owners its request for adjustment with respect to any asserted Adjustment Event, to include: (i) a detailed description of the asserted Adjustment Event, (ii) the specific reasons why the asserted Adjustment Event has or will impact Contractor’s performance, with particular focus on the specific activities on the critical path adversely affected; (iii) a schedule analysis based on the Project Schedule identifying the specific critical path activity impact(s) asserted and their extent; (iv) the asserted adjustment required to the Baseline Schedule and the Target Completion Date(s); (v) an explanation of Contractor’s mitigation efforts and strategy to minimize the impact of the asserted Adjustment Event; and (vi) relevant documentation supporting Contractor’s factual assertions.

The other Party shall promptly review such submission made in accordance with Section 11.4.3, and may request additional information or conduct its own independent review or analysis of the asserted Adjustment Event and its impact. Any adjustment to which the Contractor may be entitled for an Adjustment Event shall not include additional costs to the extent Contractor failed to implement reasonable mitigation measures to mitigate the adverse effects of the Adjustment Event.
Request Submission. If any provision in this Agreement requires a party to seek approval from the other party for any reason, the party seeking approval (the “Requesting Party”) must submit a written request for such approval (a “Request”) to the other party (the “Responding Party”) in accordance with Section 10.3 together with any materials that must be submitted with such Request pursuant to this Agreement.
