Second Medical Opinion. Members are entitled to a second medical opinion under the following conditions:
Second Medical Opinion. Where a sick leave extends beyond one (1) month or where it is anticipated at the outset that the duration of the leave will be uncertain or will last beyond one month, the University has the right to and may require a second medical opinion. The University may select either the Staff Member's nominee, or another practitioner to be mutually agreed upon between the staff member and the University. Second Medical Opinion. In cases of frequent or long term Sick Leave, the Employer may require the employee to obtain a second medical opinion from a doctor appointed by the Employer. In the event the Employer does so, the employee will authorize their doctor to make available the required information to the doctor appointed by the Employer and shall substantiate that this has been done. Such medical reports shall be treated as confidential between the employee, the doctor, and Human Resources. The cost of a second medical opinion will be borne by the Employer.
Second Medical Opinion. In cases of long term or frequent sick leave claims, the Employer may require the employee to obtain a second medical opinion from a doctor appointed by the Employer. In this event, the employee will authorize the doctor who rendered the first opinion to make the required information available to the second doctor and shall, if requested, substantiate that she/he has given this authorization. The cost of obtaining the second opinion shall be borne by the Employer. Information imparted to the University in a medical report resulting from obtaining a second medical opinion shall be retained only in the files of the University Medical Officer.
Second Medical Opinion. Within 20 days following the execution of this Agreement, the Clubs shall provide an updated, accepted listing of medical specialists, by specialty and by geographic region, to whom Players may upon their request go for diagnosis and a second medical evaluation of an employment-related illness or injury being treated by the Club Physi- cian. At least two board-certified physicians shall be designated for each specialty in each of the geographic regions, and all the physicians on the list shall be board-certified in an appropriate medical specialty. The Commissioner’s Medical Advisory Committee, in consultation with a medical professional designated by the Association, shall review and update the list of specialists on an annual basis. The Asso- ciation shall have 30 days from the date of receiving an updated list within which to recommend additions to or deletions from the list. Prior to undergoing a “second evaluation,” a Player shall inform the Club in writing of his decision to seek a second medical opinion, and the name of the physician who will be performing the diagnosis and medical evaluation. A Player may seek a “second evaluation” from a medical specialist on the accepted listing who is located outside of the geographic region within which the Player’s Club is located, provided that the Player is not absent from the Club for an unreasonable period of time. If a Player uses the services of a medical specialist who is on the accepted listing, the Club shall pay the cost of the “second evaluation,” including transportation and hotel costs. Expenses for “second evaluations” by medical specialists who are not on the accepted listing shall be authorized and paid only by prior writ- ten agreement between the Player and the Club. Second Medical Opinion. Members who have questions about their diag- noses, or believe that additional information con- cerning their condition would be helpful in deter- mining the most appropriate plan of treatment, may request a referral from their Primary Care Physician to another Physician for a second medi- cal opinion. The Member’s Primary Care Physi- cian may also offer a referral to another Physician for a second opinion. If the second opinion involves care provided by the Member’s Primary Care Physician, the second opinion will be provided by a Physician within the same Medical Group/IPA. If the second opinion involves care received from a Specialist, the sec- ond opinion may be provided by any Blue Shield Specialist of the same or equivalent specialty. All second opinion consultations must be authorized by the Medical Group/IPA. State law requires that health plans disclose to Members, upon request, the timelines for respond- ing to a request for a second medical opinion. Second Medical Opinion. If the Insured Person is diagnosed with a Specified Illness as defined under Section 12.76 or is planning to undergo a planned Surgery or a Surgical Procedure for any Illness or Injury, the Insured Person can, at the Insured Person’s choice, obtain a Second Medical Opinion during the Policy Period provided that:
a. Our Service Provider is contacted seeking the Second Medical Opinion.
b. The Second Medical Opinion will be arranged by Our Service Provider and will be based only on the information and documentation provided by the Insured Person that will be shared with the Medical Practitioner.
c. This benefit can be availed only once by an Insured Person during a Policy Year for the same Specified Illness or planned Surgery.
d. By seeking the Second Medical Opinion under this Benefit the Insured Person is not prohibited or advised against visiting or consulting with any other independent Medical Practitioner or commencing or continuing any treatment advised by such Medical Practitioner.
e. The Insured Person is free to choose whether or not to obtain the Second Medical Opinion, and if obtained then whether or not to act on it in whole or in part.
f. The Second Medical Opinion under this Benefit shall be limited to defined criteria and not be valid for any medico legal purposes.
g. We do not assume any liability and shall not be deemed to assume any liability towards any loss or damage arising out of or in relation to any opinion, advice, prescription, actual or alleged errors, omissions and representations made by the Medical Practitioner.
Second Medical Opinion. A consultation with a peer requested by the beneficiary, the HCO, a Participating Physician or the INSURER to assess the appropriateness of a previous recommendation for surgery or medical treatment. SECONDARY or SPECIALTY PHYSICIAN: A physician such as a dermatologist, urologist or cardiologist, who provides professional services on a referral from a Primary Care Provider.
Second Medical Opinion. Each covered person is entitled to request a second medical opinion from a Plan Physician with the stipulation that the Covered Person pays all applicable Co-Payments.
Second Medical Opinion. In cases of long term or frequent sick leave claims, the Division may, at its expense, require the employee to submit to an independent medical examination with a physician from a mutually agreed upon list of physicians, or where the Division and the employee cannot agree on a physician, a third party (i.e. College of Physicians & Surgeons) shall designate the physician to conduct the exam. A copy of the physician's report will be provided to the employee. Such report will be treated in the strictest of confidence by the Division.
