Service Logs a. Contractor shall maintain a Service log which describes the maintenance and repair Services provided for each Device.
b. A no-cost copy of Service logs/reports must be provided to the Purchasing Entity or Participating State or Entity, within five (5) Business Days of the request.
Service Logs. TES Personnel shall maintain auditable logs of Services provided. Such logs will be made available to Client, upon request, within five (5) business days. For purposes of this Agreement, the term “business days” shall not include Saturdays, Sundays or holidays. Should TES not be able to provide such requested Services, TES will notify Client in Writing, within ten (10) calendar days.
Service Logs. A. Franchisee shall be required to keep and maintain service logs, subject to any limitations imposed by State or Federal law, including any Subscriber privacy limitations.
B. The purpose of the service logs is to assist the City in assessing (in the aggregate) the type, degree, and resolution of Subscriber trouble calls and written complaints.
C. In addition to any other right of inspection that the City may possess, it shall have the right to review and inspect a compilation of such logs; provided, however, that Franchisee shall have no obligation to retain any log for more than three (3) years. However, the City shall not have the right of access, review, or inspection of any service logs or any information contained within service logs that are otherwise protected from access, review, or inspection by State or Federal law.
D. This Section does not require Franchisee to maintain service logs on scheduled installations, upgrades, and changes of service.
Service Logs. Provider shall timely complete and submit to School all necessary billing forms and service logs in a form reasonably acceptable to the School and its payor sources.
Service Logs. Following installation of equipment and cameras at a “Project Blue Light” site, Installer will send HPPD, on a monthly basis, service logs related to that site. Installer will also immediately provide HPPD service logs upon request. Such service logs will be provided in a format that HPPD deems acceptable, in its sole discretion.

Service Logs. District will ensure Provider has access to IEP system (e.g., SEIS, SIRAS, Service Tracker) or other documentation system used by district. Provider shall timely complete and submit to School monthly attendance roster from district documentation system (i.e., Service Tracker Attendance Roster).
Service Logs. In providing the Services, Contractor shall contemporaneously maintain accurate Service logs in a form reasonably acceptable to the District. Service logs shall include each date that Services are provided to a student, the name of the Contractor Services Provider, and a brief description of the activities or goals worked on in that session. District shall pay Contractor according to the following terms and conditions:
1. Payment for all undisputed amounts shall be made within thirty (30) days after the Contractor submits a detailed invoice to the District’s Accounts Payable Department for services actually performed.
2. Invoices must be on a form reasonably acceptable to District, submitted to an authorized District representative, and must:
a. Reference a corresponding Purchase Order number;
b. Identify the name of the Contractor Services Provider;
c. Provide a reasonably detailed description of the Services provided, identified per date, and identifying the amount of time spent in providing the Services.
3. Services shall be billed in half hour increments.
4. Services shall be performed at the following hourly billing rates: Service Rate
a. Adaptive PE Services $ 81 per hour
5. If contractor is required to travel to and perform services at more than one location, District will compensate the contractor for travel time between facilities at the regular hourly bill rate and for mileage up to the current acceptable IRS reimbursement rate.
6. District shall not be liable for any costs or expenses paid or incurred by Contractor or Contractor Services Providers in performing Services for District. Labor Code section 3700 in relevant part provides that every employer except the State shall secure the payment of compensation in one or more of the following ways: ❖ By being insured against liability to pay compensation by one or more insurers duly authorized to write compensation insurance in this State. ❖ By securing from the Director of Industrial Relations a certificate of consent to self- insure, which may be given upon furnishing proof satisfactory to the Director of Industrial Relations of ability to self-insure and to pay any compensation that may become due to its employees.
