You are not responsible if I violate those terms, or for any consequences resulting from any violation. When my Account is accessible by my mobile device, it is important that I treat my mobile device with the same care I would the Card. For example, I should secure my mobile device against unauthorized access. Additionally, if I give someone my phone, or other mobile device, that can be the same as giving that person my Card.
Signature Requirement. Employees must sign PAMs or other documentation of an incident when presented to them, such as but not limited to Attendance Tracker Documents (ATDs). This signature does not constitute an admission of guilt or agreement with the facts as stated on the document, only that the employee has been made aware of the facts.

If the employee refuses to sign the document, the supervisor will make a note that the employee has refused to sign and will place the information in the file. Signature sections shall not be on a separate page from the document. They shall be on a front to back or two-sided document to show that the signature was placed on the original document and not on a document later amended post-signature. Original signed documents shall be kept in the official personnel file at human resources.

Bargaining unit members shall be required to sign such material to be placed in their personnel file provided the following disclaimer is attached: “THE BARGAINING UNIT MEMBER’S SIGNATURE DOES NOT NECESSARILY INDICATE AGREEMENT.”
b) If a bargaining unit member is not available within a reasonable period of time to sign the material, the City may place the material in the files provided a statement has been signed by two (2) City representatives that a copy of the document was mailed to the bargaining unit member at their address of record.
