Accident and Sickness Insurance. Income benefits will be $600 per week up to 26 weeks maximum. The benefit, as noted below, will be reduced by any payment the employee is eligible to receive under the Schedule for the California Unemployment Disability Insurance Code. No deductions are made from a third paycheck in a month. Employee contributions (per paycheck where a deduction is made) at CCAFS and Decatur will be based on the total premium cost of the coverage. On each January 1, the premium will be reviewed and be increased or decreased according to past and estimated future experience as determined by the insurance carrier in accordance with accepted actuarial principles.
Accident and Sickness Insurance. Indexed to EI maximum on January 1 of each year, but not less than $555 per week effective August 13, 2018, $560 per week effective August 12, 2019 and $565 per week effective August 10, 2020. It is understood that the Company, in this year and future years, retains the total amount of premium reduction provided by the Employment Insurance Commission if the Plan meets all the requirements for registration, provided that the Accident and Sickness Benefit is indexed to the Employment Insurance Commission maximum. If you are disabled because of a non-occupational accident or illness and unable to work, a weekly indemnity benefit is payable. Benefits begin as of the 1st day of an accident or the 4th day of sickness (1st day if the illness requires you to be hospitalized or exceeds 35 days). However, should an employee not see a doctor or nurse practitioner, qualified to diagnose and treat the accident or sickness, within the first four (4) days of sickness or accident, weekly indemnity benefits will be payable only from the date of such consultation. The combination of Weekly Indemnity payments and straight time wages for hours worked during any one week shall not exceed the amount earned if an employee had worked 40 hours at his regular straight time rate. (a) Eligibility Requirements To be eligible for benefits, an employee must: • become wholly and continuously disabled by accidental bodily injury or sickness, while insured for Accident and Sickness Insurance, • be unable to perform all duties of the employee's occupation, • be under a doctor's care, and • furnish the Insurer with satisfactory proof of disability. The requirement that an employee be under a doctor's care shall be deemed to have been met if an employee under treatment for alcohol or drug abuse in a residential or outpatient substance abuse treatment facility approved for benefits under the H-S-M-D Program furnishes the insurance company with certification of disability, provided either by the facility's physician director, or by a physician consultant selected by the facility, based on information furnished by, and upon the recommendation of, the therapist who is supervising the employee's therapy. For such certification to be acceptable, the physician director or physician consultant providing it must be a licensed doctor of medicine.
