AFSCME CARE PLAN. The employer will pay into the AFSCME CARE PLAN on behalf of all full-time employees: A. $34.00 per month for Dental Level II-A insurance; B. $6.75 per month for vision care insurance; C. $1.40 per month for EAP Leavel III; and D. $.50 per month for hearing aid coverage. The payment will be due by the 20th of each month.

AFSCME CARE PLAN. Effective January 1, 2003, for each employee covered by the Agreement, who has completed thirty (30) calendar days of employment prior to the first day of the month for which contributions are to be made, 1. the College agrees to make a monthly contribution in the amount of $27.50 to the AFSCME Care Plan for full‐time employees, and; 2. $20.00 to the AFSCME Care Plan for part‐time employees for the purpose of providing Health & Welfare benefits. 3. The Union shall take whatever steps are necessary with the Care Plan administrator to procure repayment of monies paid by the College in the Care Plan for the period of time for which part‐time employees became ineligible for prescription drug coverage.

AFSCME CARE PLAN. The City shall contribute to the Ohio AFSCME Care Plan for the purpose of providing Dental IIA, Vision, Hearing, Prescription and Life Insurance benefits to eligible bargaining unit employees in accordance with the rules and regulation of the Fund and all applicable federal and state laws. Contributions shall be made between the first (1st) and the tenth (10th) of each month at the rate of sixty three dollars and seventy-five cents ($63.75) per month for each bargaining unit employee. Newly hired employees shall become eligible to enroll into the Ohio AFSCME Care Plan on the ninety-first (91st) day of employment. The monthly rate shall not increase during the term of this contract.
AFSCME CARE PLAN. A. The City shall contribute seventy dollars and seventy-five cents ($70.75) per month for each bargaining unit member to the Ohio AFSCME Care Plan. For said payment, the Ohio AFSCME Care Plan shall provide each member of the bargaining unit with the life insurance, dental insurance III, eye care insurance, and hearing care insurance benefits currently in effect in the Fund, for the duration of this contract. B. The City shall assume no responsibility for the administration of these programs.
