12.2. If you are bringing any food to the Venue, subject to clause 8, you are responsible for notifying us and your guests about any allergens and checking any potential health issues with your guests.

Allergies. If a member is allergic to animal traces or traces of certain materials, then they must inform the Manager of this when they join.
Allergies. The genetic health warranty does not cover skin/food/environmental allergies. We have no control of the things your dog may be exposed to such as the food you choose to feed, physical stresses, vaccination combinations or timing of vaccines given, or environmental factors that may trigger allergies.
Allergies. You are not covered for restorations or procedures necessary due to allergies or allergic reaction to dental treatment materials such as allergies to metals or mercury. You are not covered for local anesthesia or nitrous oxide (relative analgesia) when billed separately from the related procedure. This exclusion does not apply to general anesthesia or intravenous sedation administered in connection with covered oral surgery as described in the benefits section of this policy. You are not covered for any fees charged by your dental office because of broken appointments. Your plan does not cover the cost of cleaning removable partials or dentures. Charges for consultation are not a covered benefit. Your plan does not cover any charges to complete forms. You are not covered for services or supplies used for revision or alteration of the functional relationships between upper and lower teeth unless otherwise noted on the summary of benefits sheet at the front of this handbook. You are not covered for complications of a non-covered procedure.
Allergies. 5.1 The MUA will not be held responsible for any allergies or reactions caused by our work.
5.2 It is the Client’s responsibility to inform the MUA in writing of any sensitivities/allergies/intolerances at the time of booking or at the latest 14 days before the Event. The MUA cannot accept any liability for allergies or reactions where they have not been advised of such risks.

Please, your review your menus each week. Initials Immunization: We require that all children attending our center be immunized. It is the parent’s responsibility to provide and keep a current certificate of immunization for your child/children. Upon enrolling, parents must bring the form 3231 to the center. Each time your child is immunized, please bring a revised copy of the certificate of immunization-form 3231. Ear, Eye, Dental Form: The Learning Station requires all GA Pre-K students to submit the form 3300 (vision, dental, hearing, and nutrition screening) during the initial enrollment process. If follow up services is recommended documentation is required throughout the process. Records: I acknowledge that it is my responsibility to keep my child’s records current to reflect any significant changes as they occur, e.g. telephone numbers, work location, emergency contacts, child’s physician, child’s health status, infant feeding plans and immunization records, etc. Initials Incidents: The Learning Station agrees to keep me informed of any incidents, including illnesses, injury, adverse reactions to medications, exposure to communicable diseases, which include my child. Initials (The report is given to an administrative staff member immediately after completion.) Attire: Children should wear washable clothing which are comfortable and ok to get a little messy. Each child must have 2 changes of clothing (seasonally appropriate) infants thru 3 yrs. old need at least 2-3 changes of clothing. These clothes are to be kept at the center to be used for emergencies. All clothes should be marked with the child’s name. The center is not responsible for damaged or unmarked clothing. Initials Menus: Weekly or monthly menus are posted at the front door bulletin board and parents/guardians may refer to these at any time. Food exceptions cannot be made for individual
