Annual Clothing Issue. For the purpose of administering contract provisions dealing with annual clothing issue a standard form is provided (see Page 16-2). Early in each calendar year the Uniform Entitlement form will be completed in duplicate by the Fire Chief for each employee. The three columns "Quantity Allowed", "Prior Size" and "Date of Last Issue" will be filled in with the appropriate information. It will be the responsibility of the employee to verify the information provided to his own satisfaction. The employee will then fill in the remaining columns indicating the quantity of clothing he requests and any size changes required. One of the two copies will be returned to the Fire Chief by the employee - the other copy may be retained by the employee as a personal record. Annual Clothing Issue. 1. The annual clothing issue standard Uniform Entitlement form CFD-P-26 (the “Form”) will have the total points value reduced from the points value in effect on 2013 December 31st by the following amount for the respective Fire Department divisions:

  • Form B - Contractor’s Annual Employment Report Throughout the term of the Contract by May 15th of each year the Contractor agrees to report the following information to the State Agency awarding the Contract, or if the Contractor has provided Contract Employees pursuant to an OGS centralized Contract, such report must be made to the State Agency purchasing from such Contract. For each covered consultant Contract in effect at any time between the preceding April 1st through March 31st fiscal year or for the period of time such Contract was in effect during such prior State fiscal year Contractor reports the:

  • DHS Seal, Logo, and Flags The Contractor shall not use the Department of Homeland Security (DHS) seal(s), logos, crests, or reproductions of flags or likeness of DHS agency officials without specific FEMA pre-approval.

  • Community Outreach Please describe all community outreach efforts undertaken since the last report.

  • Leave for Association Business The Hospital agrees to grant leaves of absence, without pay, to nurses selected by the Association to attend Association business including conferences, conventions and Provincial Committee meetings and to any nurse elected to the position of Local Co-ordinator. The cumulative total leave of absence, the amount of notice, the number of nurses that may be absent at any time from one area and the number of days (including those of the Local Co-ordinator) is set out in the Appendix of Local Provisions. During such leave of absence, a nurse's salary and applicable benefits or percentage in lieu of fringe benefits shall be maintained by the Hospital and the local Association agrees to reimburse the Hospital in the amount of the daily rate of the full-time nurse or in the amount of the full cost of such salary and percentage in lieu of fringe benefits of a part-time nurse except for Provincial Committee meetings which will be reimbursed by the Association. The Hospital will bill the local Association within a reasonable period of time. Part-time nurses will receive service and seniority credit for all leaves granted under this Article.

  • Public Outreach The Sponsor is responsible for development and administration of a public outreach effort to ensure public awareness and involvement in the Project development and delivery process. The Sponsor shall provide a copy of the public outreach plan and all materials documenting the public outreach activities, including public notices, press releases, flyers, etc. to the Authority. The public outreach plan must accompany the first invoice for payment from Sponsor. The materials documenting the public outreach activities must accompany the final invoice for payment from Sponsor.
