Application and Eligibility. 15.5.1. Each eligible Occasional Teacher shall, not later than June 30 of each year, complete and return the benefits election form provided by the Board.
15.5.2. An eligible Occasional Teacher who elects to participate in one or more Employee Benefit Plans shall be a participant in the plan or plans from September 1 to the following August 31 providing that the eligible Occasional Teacher:
(i) remains on the Board's list of Occasional Teachers;
(ii) either remains available for work as an Occasional Teacher, or is absent due to illness as certified by an appropriate licensed medical practitioner; on a pregnancy or parental leave under the Employment Standards Act; or as may be otherwise permitted by the Board; and
(iii) pays the Occasional Teacher's share under 15.6.0.
15.5.3. An Occasional Teacher currently enrolled in a Plan may continue participation in that Plan from the next September 1 to the following August 31 providing that the Occasional Teacher:
(i) works at least 90 full-time equivalent school days as an Occasional Teacher with the Board during the current school year; and
(ii) fulfills the conditions set out under 15.5.2.
15.5.4. If the Occasional Teacher fails to comply with any of the conditions of 15.0.0. the Occasional Teacher's enrolment shall immediately terminate and the Occasional Teacher shall not be eligible to re-enroll except as permitted by the Plan(s) and then not until the Occasional Teacher again becomes eligible under 15.0.0. 15.6.0. Premiums
15.6.1. Monthly premium costs for each Plan in which the eligible Occasional Teacher participates shall be shared equally between the Board and the eligible Occasional Teacher.
15.6.2. The eligible Occasional Teacher's share of the premium cost shall be paid according to the procedures determined by the Board.

(b) An eligible Occasional Teacher who elects to participate in the Employee Benefit Plans shall be a participant in the plans from September 1 to the following August 31 providing that the eligible Occasional Teacher:
(i) remains on the Board's list of Occasional Teachers; and
(ii) remains available for work as an Occasional Teacher, except as may be otherwise permitted by the Board.
(c) An Occasional Teacher currently enrolled in the benefit plans may continue participation in the plans from the next September 1 to the following August 31 providing that the Occasional Teacher:
(i) works at least ninety (90) full-time equivalent school days as an Occasional Teacher with the Board during the current school year; and
(ii) fulfils the conditions set out under 8.02 (b).
(d) Notwithstanding paragraph (c) above, an Occasional Teacher currently enrolled in the benefit plans may continue participation in the plans from the next September 1 to the following August 31 providing that the Occasional Teacher:
(i) works in excess of forty-five (45) full-time equivalent school days as an Occasional Teacher with the Board during the current school year; and
(ii) fulfils the conditions set out under 8.02 (b).

Application for display space must be made on the printed form provided by the Academy of Breastfeeding Medicine (ABM, hereinafter “the Association”), contain the information as requested, and be executed by an individual who has authority to act for the applicant. This exhibition is designed for the display and demonstration of products and services relating to natural breastfeeding and those individuals attending the Association's 2023 Annual Conference. The Association shall determine the eligibility of any company, product, or service. The Association may reject the application of any company whose display of goods or services is not compatible, in the sole opinion of the Association, with the educational character and objectives of the exhibition. In the event an application is not accepted, any paid space rental fees or deposits will be returned. This contract is binding upon receipt and acknowledgment by the Association. Acknowledgment constitutes one or more of the following: Association confirmation letter or e- mail message, and shared conference information to exhibitor.

All giveaways (virtual coupon, promotional codes, etc.) to be used during the sponsored function b. Any additional digital signage, banners, etc. to be displayed during the sponsored function.

The Customer must satisfy our eligibility requirements for the Services and provide the Bank with all application information, forms, schedules, and other Documents (each, a “Schedule”) the Bank requests, in form and substance satisfactory to the Bank. Each Schedule is deemed to be incorporated in this Agreement immediately when given effect by the Bank, without any other action required by the Bank, by the Customer or by any other Person. The Bank is under no obligation to provide any Service to the Customer and the records of the Bank are conclusive as to the Services being provided to the Customer at any given time.
