Application to become a leave recipient. X. Xx Officer who has been affected by a Medical Emergency may submit written application to the Leave Bank Committee to become a leave recipient. In order to qualify for medical emergency leave, a request for leave from the leave bank may be made under the following criteria.

Application to become a leave recipient. A. A leave bank member will make written application to the Board to become a leave recipient. If a leave bank member is not capable of making application on his or her own behalf, a personal representative may make written application on his or her behalf.
Application to become a leave recipient. An application by or on behalf of an EMPLOYEE who has been affected by a medical emergency, must be made in writing to the EMPLOYER to become a leave recipient. The EMPLOYER must approve or disapprove the request and notify the EMPLOYEE in writing within 10 working days of receiving the request. The EMPLOYER may use a variety of methods to publicize the leave recipient's need for donations of annual leave.
