Chapter Responsibilities a. My chapter will report the names of all training participants to the facilitator within one week of the training.
b. No hybrid trainings - participants must be either all in person or all virtual.
c. Once training is confirmed for virtual a switch to in person is not allowed.
d. In person trainings can be converted to virtual if the ability to travel or restrictions on gatherings change. The chapter will be responsible for any cancelation fees related to venue, food and beverage.
e. In person training criteria:
1. National Level: the boarders to that country are currently open to minimum of 85% of attendees
2. Municipal Level: current laws/bylaws/rules allow for in person gatherings of that size.
f. Only in-person - My chapter agrees to provide a venue for the program.
Chapter Responsibilities. Chapter agrees to: • Submit completed Seminar Request and Logistics forms to NIGP at least ninety 90 days prior to the desired Course start date. • Provide NIGP, at no cost, a meeting room to conduct each Course. The meeting room shall accommodate seating for the number of registered attendees and shall comply with the Americans with Disabilities Act (“ADA”) requirements. Meeting rooms shall be equipped with the following: LCD projector, screen, flip chart, and markers. Chapter shall provide any additional equipment reasonably requested by NIGP in advance of the Course. • Ensure that all materials are received prior to the start of the class. • Provide on-site administrative assistance for the first hour of the first day of each Course, including but not limited to: ensuring completion of Course Participant Registration form by each attendee, attendee sign-in and Course Materials distribution. • Provide on-site administrative assistance on the last day of the class – working with the instructor on an agreed time. The onsite coordinator will handle the following duties on the last day of class: o Collect Course Participant Registration Forms, and sign in sheets, and Course Evaluation Forms and mail them to NIGP in a pre-paid return envelope provided in the course toolkit. • Collect and return all unused NIGP Materials and Textbooks to NIGP within five (5) days of the conclusion of the Course at Chapter’s sole expense.
Chapter Responsibilities. Chapter agrees to:
9.1 Submit completed Seminar Request and Logistics forms to NIGP at least sixty (60) days prior to the desired Course start date.
9.2 Provide NIGP, at no cost, a meeting room to conduct each Course. The meeting room shall accommodate seating for the number of registered attendees and shall comply with the Americans with Disabilities Act (“ADA”) requirements. Meeting rooms shall be equipped with the following: LCD projector, screen, flip chart, and markers (or equivalent). Chapter shall provide any additional equipment reasonably requested by NIGP in advance of the Course.
9.3 Ensure that all materials are received prior to the start of the class.
9.4 Provide on-site administrative assistance for the first half of the first day of each Course, including but not limited to: ensuring completion of Course Participant Registration form by each attendee as provided in the instructor toolkit; attendee sign-in and NIGP Materials distribution.
9.5 Provide on-site administrative assistance for the second half of the last day of each Course, including, but not limited to:
a. Ensuring completion of the Course Evaluation Forms by each attendee as provided for in the instructor toolkit; and
b. Collect Course Participant Registration Forms, sign in sheets, and Course Evaluation Forms, and mail them to NIGP in a pre-paid return envelope provided in the instructor toolkit.
9.6 Collect and return all unused NIGP Materials and Textbooks to NIGP within five (5) days of the conclusion of the Course at Chapter’s sole expense.
Chapter Responsibilities. The Chapter shall furnish room and board accommodations during the Term. If the Chapter House becomes uninhabitable by reason of fire, flood, windstorm or other similar casualty, the Chapter may cancel this Agreement without liability or responsibility to the Individual except as may be specifically set forth in this Agreement.
Chapter Responsibilities. (a) The Chapter must be incorporated as an Illinois not-for-profit corporation and maintain its corporate status in good standing with the Illinois Secretary of State.
(b) The Chapter must qualify to do business in the Territory in accordance with applicable state and local law and maintain its status in good standing in the Territory.
(c) The Chapter must adopt the Standardized Chapter Bylaws. Any modifications to the Standardized Chapter Bylaws must receive the prior written approval of HIMSS.
(d) HIMSS is exempt from Federal income tax under Section 501(c)(6) of the Internal Revenue Code, as amended (“Code”), and maintains a group tax exemption in order to provide its Chapters the opportunity to be recognized as exempt from Federal income tax without having to separately apply for such status with the IRS. The Chapter hereby elects to be included in the HIMSS group tax exemption, and shall take all actions necessary to maintain such tax-exempt status at all times.
(e) The Chapter shall prepare and submit all required federal, state and local corporate and tax filings.
(f) The Chapter shall hold annual member meetings, regular Board meetings, and shall conduct an annual program of activities, including regular communication with Chapter members, education, marketing, and promotion in the Territory, that meets minimum requirements set for the Chapter by HIMSS.
(g) The Chapter shall provide the following information to HIMSS annually on such dates as HIMSS may establish in its sole discretion:
(i) Completed Chapter Annual Standards Report, including all exhibits thereto (a copy of which is included in the Chapter Board Manual and available online at xxx.xxxxxxxxxxxxxxxxxxx.xxx/ ).
(ii) Copies of minutes of all meetings of the Chapter’s voting members, Board of Directors, and committees having the authority of the Board of Directors (e.g., Executive Committee, Audit Committee).
(iii) Such other information as the HIMSS Board of Directors may request from time to time.
(h) The Chapter shall assist HIMSS and provide HIMSS with all information necessary for HIMSS to fulfill its responsibilities set forth in Section 3(b).
Chapter Responsibilities. (a) The Chapter shall hold regular meetings, and shall conduct an annual program of activities, including regular communication with Chapter members, that meets the minimum requirements set for the Chapter by SAVMA. Failure to maintain the minimum number of meetings and events may result in the revocation of the Chapter’s Charter.
(b) The Chapter shall provide the following information to SAVMA annually on such dates as SAVMA may establish in its sole discretion:
(i) Copies of financial statements and records;
(ii) Copies of minutes of all meetings;
(iii) A copy of the Chapter’s meeting calendar;
(iv) Description of all activities held during the prior year;
(v) List of the Chapter’s leaders;
(vi) Leadership Affirmation Statement (a) acknowledging that the Chapter’s leaders have received a copy of this Agreement, reviewed it and understand the requirements and obligations set forth herein; and (b) representing and warranting that the Chapter is in compliance with the terms of this Agreement and will continue to abide by the terms of this Agreement; and
(vii) Such other information as SAVMA or AVMA may request from time to time. XXXXX will deliver a copy of all such information to AVMA immediately upon receipt thereof.
(c) The Chapter represents and warrants that it has not, and will not, take any action that would prevent it from being eligible for exemption from the payment of Federal income taxes as a tax exempt organization within the meaning of Section 501(c)(6) of the Internal Revenue Code of 1986, as amended (the “Code”) or that would jeopardize the AVMA group tax exemption maintained on behalf of SAVMA, the Chapter and the other SAVMA Chapters.
(d) The Chapter shall at all times conduct its activities in a manner consistent with an organization exempt from Federal taxation under Section 501(c)(6) of the Code.
(e) The Chapter shall prepare and submit all required federal, state, and local annual tax filings in a timely manner, including, without limitation, the appropriate Form 990 annual information return which must be filed with the IRS each year and shall provide copies of the same to SAVMA on an annual basis immediately upon filing. XXXXX shall deliver a copy of all such returns to AVMA immediately upon receipt thereof.
(f) The Chapter shall abide by all applicable federal, state and local laws, requirements and regulations.
Chapter Responsibilities. Chapter agrees to:
9.1 Submit completed Seminar Request and Logistics forms to NIGP at least sixty (60) days prior to the desired Course start date.
9.2 For in-person courses, provide at no cost to NIGP, a meeting room to conduct the course. The meeting room shall accommodate seating for the number of registered attendees set in pod-style seating (4-6 participants per grouping/table) and shall comply with the Americans with Disabilities Act (“ADA”) requirements. If any recommendation or restrictions are in place due to COVID-19, Chapter agrees to follow the requirements and guidelines, and comply with all local, state and federal laws. Meeting rooms shall be equipped with Wi-Fi internet, LCD projector, screen. Chapter shall provide any additional reasonable equipment requests.
9.3 For in-person courses, verify that course workbooks and supplemental materials have arrived, and that the workbook title matches the course title.
9.4 For in-person courses provide on-site administrative assistance for the first half of the first day of each course, including but not limited to, a) greeting the instructor, b) ensuring completion of course participant registration form by each attendee, c) distribute course attendee sign in sheets, and d) distribute any course materials to students.
9.5 For in-person courses, provide on-site administrative assistance for the second half of the last day of each course, including but not limited to: a) collecting course participant registration forms, and
Chapter Responsibilities. The Chapter agrees to furnish any information needed by the District to fulfill the provisions of this Article.
Chapter Responsibilities. The chapter will use the dues structure as outlined in the current APSE membership application.
Chapter Responsibilities. Chapter and University agree that:
a. Chapter will furnish necessary furniture, wall fixtures and upgrades to its Chapter Room. All project requests must be submitted in writing to Housing and Residence Life by November 1 of each year for the upcoming year to ensure that all safety measures are reviewed and there are no major renovations conducted that will change the existing structure. No reasonable request will be denied. All renovations must meet the approval of Housing and Residence Life and the Department of Facilities Administration and Planning.
b. Chapters must purchase room furnishings that meet the standards of the Alabama Fire Code.
c. Chapter is responsible for all custodial care necessary to keep the Chapter Room in a clean and sanitary condition.
d. Chapter is responsible for ensuring that the kitchens are clean properly after each Chapter event. Food and trash should be disposed of properly and general cleaning of kitchen area, including, but not limited to, cleaning dishes, counter tops, floors, etc. after Chapter events. Chapters must take trash to dumpster located next to the residence halls.
e. Should there be an excess of cleaning required in regards to Chapter functions, Chapter will be assessed a fee.
f. The name of the sorority and its insignia may be placed on the building according to plans and specifications approved by the University.
g. Chapter Room and other areas are subject to inspection by health and safety and University officials. Chapter must maintain health and safety code regulations. The University reserves the right to inspect sorority Chapter rooms and floors periodically to determine it is in reasonably neat and orderly condition.
h. Each chapter will provide a House Manager who will work with the Office of Student Engagement and Housing and Residence Life. This person will be the direct liaison between the sorority residents and the University.
