Completion of Form. All applicants for a leave of absence shall complete Appendix A-1 that is a part of this Article. The terms and conditions stated in the application shall be a part of this Article.

Completion of Form. The Contractor shall review the form and ensure that all of the blank spaces in the Request for Hearing Form are fully and legibly completed, that the person requesting a hearing has signed the form, and that each copy of the form has been completed.

For example, if employee works Monday-Friday, enter "1” . If employee works Tuesday-Saturday, enter " 2 ". The name of the second most senior employee will be placed at the top of the second column. This will be repeated until alt employees o f the same title, series and grade are listed on the same roster. (NOTE: In a few units, it may take more than one page.) Below the employee’s name, enter his/her service computation date (SCD).
Completion of Form. If evaluators are unable to secure a signature on the completed evaluation form, an email from a staff member that indicates receipt of the final evaluation form shall serve as a signature. Staff are welcome to sign the final evaluation forms and drop them off at their buildings whenever they are there.
Completion of Form. All requests for Credit Advances shall be requested by individuals authorized by Borrower. All requests for Advances will be confirmed by email, signed facsimile, or in person or by telephone with an authorized representative of Lender. If requested by Lender in connection with any request for a Credit Advance under Note No. 2, the Borrower shall complete, execute and deliver a credit request form or application (the “Form”) as a condition precedent to such Credit Advance, which Form shall be on any terms acceptable to Lender, and shall permit the Lender to obtain any reasonable information or supportive written evidence requested by Lender, all of which shall be in a form and substance acceptable to the Lender.
Completion of Form. (a) The Reseller must complete and submit the Registration Form for each applicable Grower within 30 days of an order of Seed being dispatched from Barenbrug's warehouse.
