Customer Service Liaison. Company shall designate a Customer service representative to which City can direct Customers that contact City with Solid Waste service questions. Customer Service Liaison will take responsibility for completing and closing out work orders within City’s designated work order system. Company shall also designate Customer service representatives dedicated to respond to Customer’s questions regarding the contract transition.

Customer Service Liaison. 1. Responsible for handling the State’s needs and questions. 2. Serves as a point of contact for all ad hoc requests. 3. Serves as point person for fraud investigations, subpoenas, account freezes, etc. 4. At least two (2) years’ experience managing asimilar program for a similar client.
Customer Service Liaison. The vendor shall assign not more than two (2) representatives to act as liaisons with agency representatives and Risk Management to coordinate services, scheduling and billing and to resolve problems or concerns as.they arise. The designated liaison(s) must be familiar with the contract requirements and not be assigned to a position that is subject to excessive turnover. This person or their designee will be ex ed to perfonn quality control review ofeach physical exam to ensure compliance with all reporting requirements.   • Customer Service A. PRIMARY ACCOUNT REPRESENTATIVE. Supplier will assign an Account Representative to Sourcewell for this Contract and must provide prompt notice to Sourcewell if that person is changed. Customer’s sole remedy under this section 5.2 is limited to Sage’s re-performance of the Customer Support services giving rise to Customer’s claim.

  • Autism Services This plan covers the following services for the treatment of autism spectrum disorders. • Applied behavior analysis when provided and/or supervised by an individual licensed by the state in which the service is rendered. See the Summary of Medical Benefits for the amount that you pay. • Physical therapy, occupational therapy, and speech therapy services when rendered as part of the treatment of autism spectrum disorder. A benefit limit will not apply to these services. • Psychological and psychiatric services, and prescription drugs are also covered. See Behavioral Health Services and Prescription Drugs and Diabetic Equipment or Supplies for additional information. Coverage for autism spectrum disorders does not affect any obligation of a school district, a state or other governmental entity to provide services to an individual under an individualized family service plan, an individualized education program, or similar services required under state or federal law. Services related to autism that are furnished by school personnel are not covered under this plan.
