Doctor’s Verification. 19 The District may require, as a condition of payment of sick leave, a 20 doctor’s verification following:
Doctor’s Verification. A physician's statement regarding the nature of the illness, the need for the employee's absence and the estimated duration of the absence may be required at the option of the department head or elected official for any sick leave absence after 5 consecutive days.
Doctor’s Verification. The City may require a medical release to return to work and/or a doctor’s verification of illness after a member has been absent on sick leave more than three (3) consecutive days or when a member’s pattern of sick leave use indicates potential abuse of sick leave. Any expenses, not covered by the member’s insurance, resulting from a verification of illness or a release to return to work will be the responsibility of the employer. Abuse of sick leave may be cause for disciplinary action. If the City is dissatisfied with the report of illness from the member’s physician, the City may require a medical exam from a doctor selected by the City, at the City’s expense.
Doctor’s Verification. The employee’s entire record will determine how much proof is required, as determined in the Chief of Police’s judgment and discretion. Employees with excellent attendance records may not need to prove illness while employees with poor attendance record may be required to have a doctor’s statement for each absence. Generally, employees with four (4) or more separate absences during the preceding twelve (12) months should be asked to submit a doctor’s verification.
Doctor’s Verification. A doctor's certificate shall automatically be required for sick leave requested because of illness of more than three (3) days duration.

The Chief of Police may require a verification prepared and signed by a medical doctor, describing the nature and extent of the illness or injury and confirming that the employee has fully recovered and is able to perform the full scope of the normal and customary duties of the classification. This verification shall be required when an employee is absent due to illness or injury for a period of 39 hours or more. In addition, the Chief of Police may require a medical verification any time the Chief has a reasonable basis to believe that the leave has been abused by an individual employee. Such medical verification requests shall not be unreasonably imposed.

In addition, the Chief of Police may require a medical verification any time the Chief has a reasonable basis to believe that the leave has been abused by an individual employee.
