Effective Date of Eligibility. ASES shall provide the Effective Date of Eligibility for services under the GHP to the Contractor for all Potential Enrollees as follows: Effective Date of Eligibility for Medicaid and CHIP Eligibles (see Section 1.3.1.1) is the eligibility period specified on the Form MA- 10 which is the first day of the month in which the Potential Enrollee submits its eligibility application with the Medicaid Program Office and they shall be eligible to be enrolled as of that date. For Medicaid and Chip populations, the eligibility period specified on the MA-10 may be retroactive up to three (3) months before the first day of the month in which the Potential Enrollee submits its eligibility application with the Medicaid Program Office. Retroactive eligibility is calculated independently for each of the three (3) months for which retroactive eligibility may be granted and during which services may be retroactively covered. Effective Date of Eligibility for the State Population (see Section 1.3.1.2.1) is the eligibility period specified on the Form MA-10 and they shall be eligible to be enrolled as of that date. Public employees and pensioners (see Section 1.3.1.2) shall be eligible to enroll in the GHP according to policies determined by the Government and their eligibility, Enrollment and Disenrollment processes and timeframes shall be determined through such policies. The Puerto Rico Medicaid Program and ASES do not play a role in determining the eligibility for public employees and pensioners, except in cases where the employee or pensioner seeks coverage based on income and the Medicaid Program evaluates whether income eligibility standards are met. Effective Date of Eligibility for Enrollees that have been Recertified is the date immediately following the expiration of the twelve (12) month period.

Medicaid - and CHIP Eligible Persons and members of the Commonwealth Population (see Sections 1.3.1.1, 1.3.1.2, 1. 3.1.3.1 of this Contract) shall be eligible to enroll in MI Salud as of the eligibility effective date specified on the MA-10. 4.3.4.2 Effective Date of Eligibility for Public Employees and Pensioners. Public employees and pensioners (see Section 1.3.1.3.2 of this Contract) shall be eligible to enroll in MI Salud according to policies determined by the Commonwealth. The Puerto Rico Medicaid Program does not play a role in determining their eligibility. 4.3.4.3 Certification Date is the date when the person visits the Medicaid Program Office and eligibility is determined. As of that date the Potential Enrollee may access the Covered Services. 4.3.4.4 Effective Date is the date which is up to ninety (90) Calendar Days before the Certification Date during which services can be retroactively covered for Medicaid and CHIP populations.

The Puerto Rico Medicaid Program and ASES do not play a role in determining the eligibility for public employees and pensioners.
Effective Date of Eligibility. An Employee will become an Eligible Employee on the date below upon completing the age and service requirements in B.1 and B.2a: i. Effective Date of Eligibility. 1. Initial Effective Date: All Subscribers on the Effective Date of this Contract are immediately eligible for Benefits. If Eligible Dependents of a Subscriber are covered by this Contract, their eligibility commences on the same date as the Subscriber. 2. After the initial Effective Date: For all Subscribers (and their Eligible Dependents, if dependent coverage is selected) not associated with the Contractor on the initial Effective Date of this Contract, eligibility for Benefits will begin following whichever of the following dates is applicable: a. Newly hired or rehired employees: The date for which employment compensation begins or, if applicable, that date plus the number of days specified as a waiting period in the Summary of Dental Plan Benefits. b. Spouse: Date of marriage. c. Newborn: Date of birth. x. Xxxxxx Child, adopted Child, or guardianships: Date that the Child is placed in the xxxxxx home, is placed for adoption, the guardianship becomes legally final, or the date on which the Child begins residing with the Subscriber during the waiting period for adoption or guardianship. e. Stepchild: Date that the Child’s natural parent becomes an Eligible Dependent. f. Legally Mandated Coverage: Date as required by court or administrative order. g. All others: Date that Delta Dental approves in writing the enrollment or listing of those people.
Effective Date of Eligibility. The eligibility period specified for each population covered under the GHP as described in Section 5.1.3 of the Contract.

  • Effective Date of Coverage An eligible employee is entitled to benefits provided he is actively at work on the first day the Long Term Disability Benefit Plan becomes effective. An eligible employee absent from work due to sickness or accident at the effective date of the Plan, shall only be eligible for Long Term Disability Plan benefits upon the return to continuous active full-time employment for a period of more than four consecutive weeks. The Company shall have the right to give medical examinations to employees returning from such lay-off to determine their eligibility under the Plan.
