Eye Glasses. Should an employee damage his/her glasses while on the job or because of the nature of his/her work feels that safety glasses are a requirement, he/she should first:
Eye Glasses. The Company agrees to reimburse employees two hundred ($200.00) dollars every two (2) years towards the cost of prescription glasses.
Eye Glasses. If an employee is required by his/her doctor to wear special glasses, for the sole purpose of working on a computer, the Company shall provide such glasses.
Eye Glasses. The County agrees to repair or replace prescription eyeglasses or prescription eyewear damaged or destroyed while the employee is on duty up to the deductible or $300.00, whichever is less, unless the damage is attributed to the negligence of the employee.
Eye Glasses. The Employer will provide coverage for eye glasses to $300.00 in any two consecutive calendar years. Effective January 1, 2005, the Employer will increase coverage for eye glasses to $500.00 in any two consecutive calendar years.
Eye Glasses. The Company agrees to pay one hundred percent (100%) of the monthly premiums for prescription eye glasses subject to the conditions of the Carrier. The maximum amount payable in any twenty-four (24) month period is four hundred ($400) this includes eye exams. Effective March 1, 2017 the maximum payable in any twenty-four (24) month period is four hundred ($425) this includes eye exams.
Eye Glasses. $110.00 will be paid by the U.G.W. Benefit Fund toward eye glasses, once every two years, to be paid to employees of a contributing Employer only. Qualifications are: employee must be a Union member in good standing for one (1) year and have Company seniority for one (1) year. Payment will be made when the Employee has:
Eye Glasses. (ACA&GEN) An employee who is engaged in computer/VDU duties or undertaking fine detail/precision work for at least half of a full-time position shall be entitled to a contribution to the supplier of up to a maximum of $100 towards an eye test after the first year upon being assigned to such duties. Further tests shall be provided at not less than two yearly intervals upon application to the Employer. If the test discloses that eye glasses are required for the normal viewing distance of a computer/VDU, or undertaking fine detail/precision work, then the Employer will pay a maximum contribution to the supplier of:
Eye Glasses. 14.8.1 The ICCR shall pay up to fifty ($50) dollars for an eye examination and up to one hundred ($100) dollars for new eyeglass lens costs annually for those employees who are regularly required to operate a video display terminal (VDT), upon submission of a copy of the doctor’s exam and paid bills. Employees must first submit claims to their insurance carrier. Unused amounts may be rolled over to a maximum of one hundred ($100) dollars for eye exam and two hundred ($200) dollars for lenses in any two (2) year period.
