The numeric component of the course-faculty evaluations may be relied upon as part of the Annual Performance Review. Anonymous material produced by students, as part of the course-faculty evaluations, shall be provided to the Xxxx, but shall not be relied upon as part of the Annual Performance Review. Anonymous material may only be relied upon by the Xxxx in order to discuss career development and improvement opportunities with the Faculty member.
File Contents. 10 The following provisions apply to personnel files. No document containing derogatory material or 11 making reference to a teacher's competence, character, or manner will be kept or placed in the 12 teacher's personnel file: a) without the teacher's knowledge; and b) unless it has been investigated 13 and found to be true and accurate. Each teacher has the right to ask for a hearing with the 14 Superintendent concerning review and removal of a document or documents placed in the 15 teacher's personnel file.

File Contents. Only appropriate information relating to performance or discipline may be entered into the file other than routine financial or demographic data. All such performance information must be signed by the employee’s supervisor.

The official personal history folder shall contain all memoranda and documents relating to the employee which contain criticism, commendation, appraisal, or rating of the employee's performance on the employee’s job. Copies of such memoranda and documents, with the exception of letters of reference, shall be sent to the employee simultaneously with their being placed in the official personal history folder.

No anonymous letter, report, or communication shall be included in an employee's personnel file. If an administration communication or communication received from parents and other nonprofessionals regarding an employee is intended to become part of the file, it shall be reviewed with the staff member involved, and the staff member shall be afforded the opportunity to file a written reply. No communication from another member of the bargaining unit, other than a supervisor (if countersigned by a non- bargaining unit supervisor), may be entered into the file. The employee shall initial and date each such document, other than routine financial or demographic data (e.g., change of name, address, etc.), entered into his/her file to verify its review.
