Health, Disability and Life Insurance Benefits. A. The City agrees to provide and to pay the premiums for health, hospitalization, surgical, vision, dental, and comprehensive medical insurance for the City Manager and her dependents, at a minimum, equal to that which is provided to all other employees of the City.
B. The City agrees to put into force and to make required premium payments for disability coverage for the City Manager.
C. The City shall pay the amount of premium due for term life insurance in the amount of two (2) times the City Manager's annual base salary, including all increases in the base salary during the life of this Agreement.

EMPLOYEE shall be eligible to receive basic life and accidental death and dismemberment consistent with that which is provided to all other department heads in attachment “A” (DEPARTMENT HEAD Benefits). EMPLOYEE shall be eligible to receive health, vision dental and comprehensive medical benefits consistent with those offered to all County employees.

C. The Employer agrees to provide and to pay the premiums due for term life insurance in the amount of at least two (2) times the Employee’s annual base salary, including all increases in the base salary during the life of this agreement. The Employee shall name the beneficiary of the life insurance policy.
D. The Employer shall provide travel insurance for the Employee while the Employee is traveling on the Employer’s business, with the Employee to name beneficiary. Should the Employee die while on travel for the Employer, the Employer shall cover the full cost of retrieving and transporting the Employee’s remains back to the custody of the Employee’s family.

The Employer agrees to provide and to pay the premiums for health, hospitalization, surgical, and comprehensive medical insurance for the Employee equal to that which is provided to all other employees of the City of Lakeland. Employee shall pay for dependent coverage on the same basis as all employees.
B. Employee shall be provided the same long-and short-term disability coverage as all other City employees.

Notwithstanding any other provision in this agreement, in the event that the employment of the Employee is terminated after a change in control (unless the Employee is terminated for cause), beginning as of the last day on which the Employee is afforded medical, dental, health, disability and life insurance benefits under Section 3 and/or 8 of this agreement, the Employee shall be entitled to purchase from the Association, on a monthly basis for a period of up to five years, medical, health and/or life insurance benefits of the same type then available to the Association's executive employees at an annualized price equal to the product of (i) a fraction of the numerator of which is one and the denominator of which is the number of the Association's employees at the end of the year and (ii) the Association's total annual cost for all employees for such benefits.
Health, Disability and Life Insurance Benefits. A. Employee shall participate in the City’s health benefit (medical, dental, vision, employee assistance) plans in accordance with the terms then applicable to other non-represented employees of the City. Such terms change from time to time, and shall not be more generous than the terms applicable to other non-represented employees. For example, in 2018, the Employer will provide 95% of the medical plan premium for the Employee’s coverage, and 85% of the premium for medical coverage for a spouse/domestic partner or other dependents.
B. The City shall provide long-term disability insurance for Employee with terms equivalent to that which applies to City non-represented employees.
C. The City shall pay one hundred percent (100%) of the premium for term life insurance in the amount of Three Hundred Thousand Dollars ($300,000). The Employee shall name the beneficiary(ies) of said term life insurance.

The Employer agrees to provide and to pay the premiums for health, hospitalization, surgical, vision, dental and comprehensive medical insurance for the Employee and his/her dependents equal of that which is provided to all other employees of the County. The Employer shall pay the amount of premium due for term life insurance in the amount provided to all other employees, including all increases in the base salary during Employee’s employment. The Employee shall name the beneficiary of the life insurance policy. Upon reasonable notice, Employer shall have the right to amend, modify, and/or terminate the foregoing benefits at any time, in its sole discretion. If Employer reduces the base salary, compensation or any other financial benefit of the Employee, unless it is applied in no greater percentage than the average reduction of all department heads, such action shall constitute a breach of this Agreement and may be regarded as a termination and subject to the severance benefits outlined in Section 11.
