Leave Authorization. The employee's request and the Co-operative's decision concerning all leaves of absence referred to in this article shall be made in writing.

Leave Authorization. (1) An employee may be granted up to thirty (30) workdays unpaid leave at the discretion of the appropriate Deputy/Associate/Assistant Superintendent.
(2) Unpaid leave beyond thirty (30) workdays may be granted at the discretion of the Employer.
(3) All unpaid leave requests must be received ten (10) business days prior to requested date of leave.
Leave Authorization. The District may require written medical verification by the bargaining unit member’s physician, medical consultant, or licensed practitioner prior to, during, or following any leave taken in this article.
Leave Authorization. When employee illness results in an absence of more than two calendar weeks, employees may be required by the Employer to have an examination at the Employer's expense. A competent medical authority of the Employer's choice will determine the need for continued absence.
Leave Authorization. Absence from work with full pay and benefits, or with no pay and no benefits, is authorized under various conditions outlined below. Except in the case of a bona fide emergency, advance approval from the Chief of Police or his/her designee is required. If the Chief of Police or his/her designee is not contacted during the first hour of a regular work schedule, an employee may be considered as on an unauthorized absence and carried in a non-pay status. If an employee fails to return to work upon the expiration of an authorized leave of absence, such action will be considered a resignation and all rights and benefits are terminated unless the employee has an excuse acceptable to the University.

Upon request from the employee, the Employer will provide written confirmation or denial of the leave, with reasons, within five (5) working days, of said leave request.
