Leave With Pay for Personal and Family-Related Responsibilities. (a) For the purpose of this clause, family is defined as spouse, dependent children (including xxxxxx children or children of spouse), parents (including step-parents or xxxxxx parents) or any relative permanently residing in the employee’s household or with whom the employee permanently resides. (b) NAV CANADA shall grant leave with pay under the following circumstances: (i) up to one (1) day for a medical or dental appointment to accompany a dependent family member when the dependent family member is incapable of attending the appointment by himself or herself, or for appointments with appropriate authorities in schools or adoption agencies. An employee is expected to make reasonable efforts to schedule medical or dental appointments for dependent family members to minimize his or her absence from work. An employee requesting leave under this provision must notify his or her supervisor of the appointment as far in advance as possible; (ii) up to three (3) days of leave with pay to provide for the temporary care of a sick member of the employee’s family and to provide an employee time to make alternate care arrangements where the illness is of a longer duration; (iii) one (1) day of leave with pay for needs directly related to the birth or to the adoption of the employee’s child. This leave may be divided into two (2) periods and granted on separate days. (iv) up to one-half (1/2) day for the employee in order to attend personal medical, dental or legal appointments. (c) The total leave with pay which may be granted under sub-clauses (b)(i), (ii), (iii), and (iv) shall not exceed five (5) days in a leave year. (d) The employee shall demonstrate the need for such leave in such manner as may be reasonably determined by NAV CANADA.

(b) NAV CANADA shall grant leave up to five (5) days with pay in every leave year, to be taken in one or more periods, for employees under the following circumstances: (i) personal medical, dental or legal appointments, (ii) treating their illness or injury, (iii) carrying out responsibilities related to the health or care of any of their family members, (iv) carrying out responsibilities related to the education of any of their family members who are under 18, (v) addressing any urgent matter concerning themselves or their family members, (vi) attending their citizenship ceremony under the Citizenship Act, (vii) for needs directly related to the birth or to the adoption of the employee’s child, (viii) any other reason prescribed by regulation. (c) An employee requesting leave under this provision must notify their manager of the appointment as far in advance as possible.

  • Resident Responsibilities The Resident agrees to pay all fees specified, to observe all rules and regulations of the University of Connecticut and to abide by the Responsibilities of Community Life: The Student Code, this contract and any addendum, as well as other University publications/policies. Residents assume total responsibility for their room/suite/apartment/house and for the behavior and activities which occur within all assigned living areas. Applicants and/or residents cannot exchange money or favors for a room assignment. Failure to fulfill the terms of the above may lead to termination of this contract, removal from on-campus housing, and a community standards process resulting in a sanction, including but not limited to expulsion.

  • Administrative Responsibilities Client shall be responsible for orienting Consultant to Client’s policies and procedures regarding the submission of any requisite paperwork which must be tendered for reimbursement by funding entities such as Medicare, Medicaid, or health insurance. Such paperwork may include, but is not limited to, patient care plans, comprehensive patient histories, individual education plans, or Client specific program plans. During the contracted assignment, should Consultant fail to submit paperwork as required per Client’s policies and procedures, Client must notify Soliant in writing within three (3) business days of alleged failure. Failure to notify Soliant before assignment ends shall negate any Client claim to withhold payment due to untimely work and/or paperwork non-compliance by Consultant. Client agrees that all approved time sheets by Client’s assigned representative are not subjected to billing dispute if Client fails to notify Soliant of time sheet and work performed discrepancies.

  • Student Responsibilities The school provided Chromebook for the student is an important learning tool to be used for educational purposes. In order to use the device each day, the student must be willing to accept the following responsibilities: ● I understand that district officials have the ability to monitor my use of the device AT ALL TIMES IN AND OUT OF SCHOOL and that communications, files, internet search activities, and any other actions using the device are not considered to be private. Note: RCCSD does not have the ability to and will not remotely operate the camera on the device. However, students can cover it when not in use to ensure others are not. ● I understand that the device assigned to me is on loan from Red Clay Consolidated School District.
