Membership Card. 4.1 Membership cards are issued to all Members and must be shown at the Club to gain entry on each visit made to the Club or its golf course(s).
Membership Card. 4.1 A membership card will be issued to each Member when they join a centre.
Membership Card. The Employer shall include a Union Membership Card in each employee’s employment paperwork. The card will be reserved for the Union Representative/Advocate, as available, to review the membership card with new employees during their orientation. After collecting said card from the new employee or Advocate, the Employer shall retain a copy for itself and send the original to the Union.
Membership Card. Alternative Member and any of its Related Parties over nine (9) years old must present to HRCA a Membership Card for access to and use of the Recreation Facilities. Alternative Member and each such Related Party will be required to purchase a Membership Card from HRCA at the then-current cost being charged by HRCA to the HRCA Members. If a Membership Card is lost, stolen, destroyed, or otherwise must be replaced for any reason, the Alternative Member will be required to purchase a replacement Membership Card, at the then current charge being imposed by HRCA. Guests of Alternative Member will be issued a pass upon payment by Alternative Member of the guest fee. The fees for the purchase and issuance of Membership Cards are required to be paid in addition to the Annual Fee.
Membership Card. 1. After the Member has paid the Membership Fee (or its proportional amount), the Member will be issued with a Membership card. The Membership card is a proof of Membership and entitles the Member to use the Services.
Membership Card. An integrated Medicare and Medicaid membership card that conforms to the requirements in Minnesota Statues, § 62J.60 subd. 3, and has been approved by the STATE prior to printing, that identifies the Recipient as an MCO Enrollee and contains an MCO telephone number to call regarding coverage, procedures and Grievances and Appeals. The membership card shall demonstrate that the Enrollee is a Recipient of Minnesota Health Care Programs, either by printing the Enrollee’s STATE PMI number on the card, or by other reasonable means. The card may include data elements required by CMS for Medicare eligible Enrollees.
Membership Card. 1. After making the Membership Agreement and paying the Fee, the Member receives from the Operator his or her membership card (the “Card”) for the Member to prove his or her identity to get access to the premises of the Fitness Centre.

Membership Card. Upon joining, members will be issued with a membership card which must be signed to activate it and be assigned a PIN. This card must be presented to enable members to borrow items, place holds on items and access other library services where required. The membership number and PIN will be required for online access to electronic resources and member’s library records. Members are responsible for the safe keeping of membership cards, the library is unable to provide storage of cards. The membership card is supplied for the exclusive use of the member only and is not transferable to another person. The library accepts no responsibility for use of a membership card and members remain responsible for any loans or other services accessed using their cards. Any loss or theft of a membership card must be reported immediately to prevent unauthorised use. Replacement cards are available from any of the libraries for a fee, upon confirmation of identity with a document listed at paragraph 5. The library must be notified as soon as possible of any change of address or other details of membership. Proof of residence will be required. The membership card is valid for a set period and library staff will advise the member when the card is due for renewal and ask for verification of member details and the payment of any outstanding fees attributable to the card.
Membership Card. It will be issued at the time of registry/possession whichever is earlier for using all the above facilities/amenities.
