Prescription Safety Eyewear. The Company will pay 100% of the cost of prescription safety glasses and frames (excluding any examination fees) as required by an employee through a vision care program every two calendar years. The frames selected must be from the pre-approved list established by the Company. If an employee sustains damage to the prescription safety glasses while performing his / her assigned duties, the Company will pay 100% of the cost of repair or replacement of the glasses based on the foregoing provision. Note: • Should an employee currently wear progressive bi-focals they will be able to purchase the prescription safety glasses with the same type of lenses; • When an employee selects their first pair of prescription safety glasses, they will be able to also select a pair of prescription safety sunglasses if they require two sets due to needing clear and shaded lenses safety glasses. This will require managerial approval prior to purchasing the safety glasses; • An employee who selects both the prescription clear and shaded safety glasses will not be able to select another pair of prescription safety glasses until 36 months have passed unless their current prescription safety glasses have been damaged while performing his/her assigned duties.
Prescription Safety Eyewear. The Employer agrees to reimburse any employee 50% of the cost of prescription safety glasses (contact lenses are not eligible) up to $300.00 according to the following criteria:
a) The employee must provide a copy of the prescription and receipt for the glasses.
b) The employee must have worked 1200 hours with the Employer for the first reimbursement.
c) For any subsequent reimbursement the employee must have worked an additional 4000 hours from the last time reimbursed.
Prescription Safety Eyewear. The Employer agrees to reimburse any employee fifty percent (50%) of the cost of prescription safety eyewear that is compliant with the Employer’s safety eyewear policy up to three hundred dollars ($300.00) according to the following criteria. The employee must have worked 150 hours with the Employer for the first reimbursement. For any subsequent reimbursement the employee must have worked an additional 4000 hours from the last time reimbursed.
Prescription Safety Eyewear. The Employer agrees to reimburse any employee fifty percent (50%) of the cost of prescription safety eyewear up to two hundred and fifty dollars ($250.00) according to the following criteria: The employee must provide a copy of the prescription and receipt for the safety glasses. The employee must have worked twelve hundred (1,200) hours with the Employer for the first reimbursement. For any subsequent reimbursement the employee must have worked an additional four thousand (4,000) hours from the last time reimbursed.
Prescription Safety Eyewear. The Employer agrees to reimburse any employee fifty percent (50%) of the cost of prescription safety eyewear that is compliant with the Employer’s safety eyewear policy up to three hundred dollars ($300.00) according to the following
Prescription Safety Eyewear. The Employer agrees to reimburse any employee fifty percent (50%) of the cost (up to a maximum reimbursement of two hundred and fifty dollars ($250.00) for prescription safety eyewear subject to the following criteria. The employee will be eligible after four thousand (4,000) hours of employment and every subsequent four thousand (4,000) hours thereafter. In the case of a break in employment of more than six (6) months or where the employee quits the time requirements will have to be re-served. Should an eligible employee elect to have corrective laser eye surgery, they are eligible to receive a one-time payment of two hundred and fifty dollars ($250.00) subject to the above mentioned criteria.
Prescription Safety Eyewear. The Company agrees to reimburse any Employee fifty percent (50%) of the cost of prescription safety glasses (contact lenses are not eligible) up to two hundred-fifty dollars ($250.00) according to the following criteria:
(a) The Employee must provide a copy of the prescription and receipt for the glasses.
(b) The Employee must have worked one thousand two hundred (1200) hours with the Company for the first reimbursement. The Employee must provide a copy of their pay stub showing the 1200 hour qualifying hours have been met.
(c) For any subsequent reimbursement the Employee must have worked an additional four thousand (4000) hours from the last time reimbursed.
Prescription Safety Eyewear i. The Company shall provide non-prescription and prescription safety eyewear to all employees whose jobs involve potential eye hazards. Employees are responsible for wearing safety glasses whenever their jobs involve potential eye hazards.
ii. New prescription safety lenses, and if necessary, frames, will be provided for eligible employees when an employee's corrective vision prescription warrants the change.
iii. The Company will purchase safety eyewear through an approved safety equipment supplier selected by the Company. Procedures for the purchase of prescription safety glasses will be issued by the Procurement Department
1. Each employee will be responsible for providing a current prescription (no more than one year old) from his or her optometrist or ophthalmologist at the time the eyewear is ordered. The cost of eye examinations will be paid by the employee.
2. Only prescription safety eyewear meeting ANSI requirements for industrial safety glasses will be used. Safety & Environmental Affairs Policy Manual Personal Protective Equipment Page: 1 of 5 Safety eye protection will be available for all visitors, vendors and contractors entering eye protection areas.
Prescription Safety Eyewear. For employees who do not have this coverage through another source, the Employer agrees provide reimbursement of up to $250.00 per year for the purchase of prescription for safety eyewear provided by Securo Vision Inc. To be eligible, an employee must have worked for the Employer a minimum of 180 hours during the previous 12 months.
Prescription Safety Eyewear. Provided the CEP Health and Welfare Fund benefits do not provide coverage for Employee eyewear, Employers will reimburse any Employee 50% of the cost of prescription safety eyewear up to $200 according to the following criteria:
(a) The Employee must have worked 1200 hours with an Employer for the first reimbursement; and,
(b) For any subsequent reimbursement the Employee must have worked an additional 4000 hours from the last reimbursement.
