On my part, I will take responsibility to create a supportive environment, give timely and helpful feedback, and be available as needed. As a supervisee, you will be expected to be an active participant in the supervision process, be open to feedback, be truthful and share mistakes, take responsibility for correcting any actions that could harm clients, be prepared and on time for each session, and keep proper client documentation. Professional Disclosure. I, Xxxx Xxxxxx-Xxxxx (supervisor), earned my Master of Social Work at California State University, Long Beach (CSULB) in 2008, earned my Bachelor of Social Work at CSULB in 2005, and I am licensed by the Illinois Department of Financial and Professional Regulation. I have been licensed as an LCSW since 2019 and I am a member of the National Association of Social Workers. I have provided supervision to MSW Interns in Illinois and California (as a MSW task supervisor) and provided clinical supervision for LCSW candidates in Illinois since 2021 and LCPC candidates since 2022. My clinical experience has focused on trauma, depression, anxiety, child welfare, sexual violence, intimate partner/domestic violence, community violence, aging, end of life, and grief. I approach therapy and supervision from a trauma informed, eclectic, patient centered approach. I utilize motivational interviewing, solution-focused, CBT, TF-CBT, EMDR, and Brainspotting in my clinical work and have additional training in the Xxxxxxx Method. Practical Details Individual supervision is to be scheduled between the supervisor and supervisee and is to occur weekly for one hour. Supervisees are responsible for tracking their own hours. While attendance will be tracked, it is the supervisee’s responsibility to track the number of hours needed for licensure. Cancellations must be made at least 24 hours in advance by either phone, e-mail, or text. If the time can not be rescheduled within the time frame needed to comply with individual licensing bodies, the time for that week will not be counted toward licensure hours. No shows will be charged the full amount of the session and hours will not count toward licensure hours. Digital/card payments are accepted via Square. Rates for 2023 are $75 per hour for individual supervision. In the event the supervisee wishes to switch to group from private supervision, this request will be accommodated once there is space within the group and in accordance with the governing body’s supervision hour timelines.
