Statement of Absence. Employees taking less than three (3) days sick leave shall complete NPD Form 700, "Statement of Absence." This form shall be submitted to the attention of the employee's Bureau Commander within two (2) days after returning to work.

Statement of Absence. On the last day of each month, each employee shall complete and deliver to the District a Personal Absence affidavit stating the dates and causes of the absences for that month. In addition, after five (5) consecutive days of absence taken under the provisions of Section A and upon the request of the District, the employee shall obtain and deliver to the District a written doctor's statement verifying the reason for the absence, the nature of the disability and the duration of the absence. The District shall notify the employee, in writing, within twenty (20) days of any leave taken that does not qualify under this section.
