TRUSTEES, THROUGH THE UNIVERSITY, SHALL. A. Designate the students who are enrolled in the Nursing Program of the University to be assigned for clinical nursing experience at the Facility.
B. Be responsible for all instruction and evaluation of student performance required to meet the course objectives given at the FACILITY to the students so designated.
C. Be responsible for keeping all attendance and academic records of the students.
D. Provide guidance to students in their clinical activities, through an individualized Learning Contract which specifies learning activities to take place within the patient care framework of the FACILITY.
E. Agree that the students and instructors shall be subject to the requirements and restrictions specified jointly by representatives of the UNIVERSITY and FACILITY, and subject to the FACILITY's rules and regulations governing conduct.
TRUSTEES, THROUGH THE UNIVERSITY, SHALL. A. Designate the students who are enrolled in the San Xxxx State University's Practicum Programs of the MS Clinical Program to be assigned for field experience at the Facility, in such numbers as are mutually agreed to by both parties;
B. Monitor all instruction for field work experience given at the Facility to the assigned students.
C. Keep all attendance and academic records of students participating in said Program; and
D. Require every student to conform to all applicable Facility policies, procedures, and restrictions specified jointly by representatives of the University and Facility.
TRUSTEES, THROUGH THE UNIVERSITY, SHALL. A. Designate the students who are enrolled in the Human Services Program of the University to be assigned for learning experience at the Facility.
B. Be responsible for keeping all attendance and academic records of the students.
C. Provide guidance to students in their activities, through an individual and class setting which specifies learning activities to take place within the framework of the FACILITY.
D. Upon request, provide insurance for general liability coverage.
TRUSTEES, THROUGH THE UNIVERSITY, SHALL. Designate the students who are enrolled in the San Xxxx State University's Practicum Programs of the MS Clinical Program to be assigned for field experience at the Facility, in such numbers as are mutually agreed to by both parties;
TRUSTEES, THROUGH THE UNIVERSITY, SHALL. A. Designate the students who are enrolled in the San Xxxx State University’s Practicum Programs of the School of Social Work to be assigned for field experience at the Facility, in such numbers as are mutually agreed to by both parties;
B. Monitor all instruction for field work experience given at the Facility to the assigned students and provide the necessary Social Work instructor(s) when the Facility field supervisor is not a Social Worker;
C. Keep all attendance and academic records of students participating in said Program;
D. Require every student to conform to all applicable Facility policies, procedures, and restrictions specified jointly by representatives of the University and Facility; and
E. Require University’s School of Social Work instructors to notify Facility in advance of:
1. Student(s) assigned to the facility.
2. Student Schedules 3. Changes in placement assignments
TRUSTEES, THROUGH THE UNIVERSITY, SHALL a. The UNIVERSITY shall be responsible for the selection, placement, and/or removal, and final grading of students placed with the FACILITY. These decisions shall be made in consultation with the FACILITY in accordance with the respective responsibilities of each party of this agreement.
b. The UNIVERSITY shall assign a representative of its faculty to act as Faculty Field Liaison whose responsibilities shall be to act as liaison between UNIVERSITY and FACILITY in the development and execution of the Fieldwork Instruction program and the valuation of student performance, and to engage in such other activities as are of mutual concern in the provision of student training.
c. The UNIVERSITY shall, at the time of agreement, provide the FACILITY with a statement of its educational goals, of appropriate learning experiences, and of its expectations for student performance in the practicum.
d. Upon request, provide insurance for general liability coverage.
TRUSTEES, THROUGH THE UNIVERSITY, SHALL. A. Designate the students who are enrolled in the Masters Marriage and Family Therapy Program of the University to be assigned for clinical experience at the AGENCY.
B. Be responsible for all instruction and evaluation of student performance required to meet the course objectives given at Agency to the students so designated.
C. Be responsible for keeping all attendance and academic records of the students.
D. Agree that the students and instructor shall be subject to the requirements and restrictions specified jointly by representatives of the UNIVERSITY and the AGENCY, and subject to the AGENCY’S rules and regulations governing conduct.
E. Upon request, provide insurance for general liability coverage.
TRUSTEES, THROUGH THE UNIVERSITY, SHALL. A. Make an official request to the Facility for placement of a student or students within the Facility for the student's fieldwork experience.
B. Keep all academic records of students participating in said program.
C. Designate a staff member to participate with the Facility designee in implementing and coordinating the program of supervised field placement.
D. Require every student to conform to all applicable Facility policies, procedures, and regulations, and all requirements and restrictions specified jointly by representatives of the University and Facility.
E. In consultation and coordination with the Facility's Director and staff, plan for the internship experience to be provided to students under this Agreement.
F. In consultation and coordination with the Facility's Director arrange for periodic conferences between appropriate representatives of the University and Facility to evaluate the internship experience provided for under this Agreement.
