If a Member is continuously disabled on the day this Agreement is terminated by the Plan, benefits will be provided under the terms of this Agreement while the Member is so continuously disabled. For purposes of this Subsection, continuously disabled means that the Member is an Inpatient in a Facility Provider. BENEFITS AFTER TERMINATION OF COVERAGE. 1. If an Insured's coverage terminates for any reason, Benefits under this Contract shall cease as of the effective date and time of such termination. However, if the termination of coverage is caused by failure to pay premiums on behalf of the insured, a thirty-one day grace period will be observed on behalf of the plan. COST OF THE PLAN • the University pays the full cost of the first $25,000 of coverage As the University provides a Sick Leave Benefit that qualifies for a reduction in Employment Insurance Premiums, a portion of the reduction (5/12) is used to pay the cost of an additional amount of the Basic Life Insurance plan. Currently this reduction amount provides sufficient funding for an additional $19,000 of coverage. BENEFITS AFTER TERMINATION OF COVERAGE. If you are an inpatient on the day your coverage under this Program terminates, benefits of this Program will continue until the earliest of the following:  Maximum benefits have been received;  The inpatient stay ends; or  You become covered for the condition for which you are receiving inpatient care under any other group coverage.

However, if the termination of coverage is caused by failure to pay premiums on behalf of the Insured, a forty-five (45) day grace period will be observed on behalf of the plan. During this grace period, the Contract shall remain in force. If past due premiums are not paid within the forty-five (45) day grace period then the Insured’s coverage will be terminated with an effective date equal to their last premium “paid to” date. Any claims paid for service after the termination date will be pursued for refunds.
