Credit Application. The Customer authorises Supplier, its authorised agents or representatives, to make enquiries as to the credit and financial history and responsibilities of the Customer, and/or its Representative (as applicable), as required by Supplier from time to time. If Supplier so requires, the Customer must provide (and must procure that its Representatives or guarantors, as applicable, provide) any consent or authorisation required by law for Supplier, its Representatives to undertake any such enquiries, including obtaining reports from credit reporting bodies as to the consumer and/or commercial creditworthiness of the Customer and/or its Representatives or guarantors (as applicable). Any credit terms granted to the Customer is on the condition that Customer has made complete and accurate disclosure to Supplier in its credit application of all material information relevant to Supplier’s decision to extend credit. The Customer must promptly inform Supplier of any material adverse change in Supplier’s financial or business circumstances.

Credit Application. Any new customer must obtain our credit approval before we will arrange for transportation. The following information must be submitted to our Credit Department: (a) A completed credit application signed by an owner (in the case of a sole proprietor or partnership) or an authorized officer (in the case of a legal corporation or limited liability corporation) of the prospective customer. (b) “Bill To” information, including physical address, phone number, e-mail, and contact name of the customer.
Credit Application. Process. Customer must complete two steps in order to qualify for an SLA credit. First, Customer must open a Trouble Ticket in response to Service issues. This first step brings the problem to the attention of Verizon customer service for intervention and repair, as required. The second step is to request the credit in writing from Customer’s account team contact. The timing of the written request varies by Service Level Standard and is described below. The account team receiving the SLA credit request will confirm receipt with Customer by either email or fax. Verizon will then investigate the outage through the Trouble Ticket history and notify Customer of the outcome of the investigation either by email or fax.

Credit Application. (A) Credits to apply to lanes beyond a particular degree lane must be earned subsequent to the earning of the degree and must be taken at an institution accredited by the Council for the Accreditation of Educator Preparation (CAEP), except as otherwise approved as an exception by the Superintendent. (B) It is understood and agreed by both parties that credits not reported to the District within one (1) calendar year following completion of the courses for which the credits were earned shall not be considered applicable for lane change purposes. (C) Prior to the execution of the teacher's first teaching contract with the District as a licensed teacher, the teacher shall report to the District, in writing, all credits earned prior to the start of employment in the District beyond those required for the teacher's initial license. This report shall be made in the form of an official transcript from the college or university where these courses were completed. This report shall be made available to the District prior to final execution of the teacher’s first contract with the District. Credits not reported in accordance with this requirement will not be eligible for consideration for lane change purposes. Any credits earned beyond a bachelor’s degree, and required to obtain licensure, may be applicable to initial placement on the salary schedule upon approval of the Superintendent. (D) For teachers employed prior to the final execution of this agreement, credits earned to acquire a teaching license shall not be applicable for lane change purposes unless earned at the written direction of the District. Credits earned to fulfill state licensure requirements shall be applicable for lane change credit providing written request is made to the Superintendent prior to enrollment in the course and same is approved. Credits may involve video courses, correspondence work, online courses, or self-study at the sole discretion of the Superintendent.
Credit Application. To apply on the salary schedule, all credits beyond the bachelor's degree must be graduate credits and must be in addition to those credits which are necessary for initial certification. All credits will be on a quarter credit basis (i.e., 1.5 x semester credit). Also, credits used toward advancement on the salary schedule must also be earned by completing courses relating to the major teaching field, or as a part of a definite career plan in education, which has been previously approved by the Executive Director of Administrative Services. A portion of acceptable credits may be credits earned from experience designated as "in-service" by the Executive Director of Administrative Services and/or a committee appointed by the Executive Director of Administrative Services for that purpose. New areas of licensure (district required): 1. Upon such a request by the district, the teacher shall be given 3 school years to complete the necessary coursework. 2. Any employee that has no option of lane advancement will be reimbursed by the district for actual cost of tuition and fees related to all coursework. 3. Any teacher wanting to utilize this provision has the responsibility to submit to the Human Resources office the following: request for pre- approval of courses, official transcripts upon completion of courses, proof of payment, and a copy of the new license. 4. This section shall not apply to changes in state licensure requirements or licensure change voluntarily pursued by the teacher.
