Definition of Dependent. The definition of a dependent shall include the domestic partner of an employee and the dependents of such domestic partner. Any employee claiming a domestic partner and/or the dependents of such domestic partner for purposes of this Article shall have an approved City Affidavit of Domestic Partnership form or a registered State of California Declaration of Domestic Partnership form on file in the Employee Benefits Office, Personnel Department, which identifies that individual as the employee’s domestic partner.

For the purpose of this Article, dependent shall be defined as an individual dependent child (natural child, step child, or adopted child) who is actually claimed as a personal exemption under the provisions of Section 151 of the Internal Revenue Code, a full time University of Toledo student (minimum of twelve credit hours) and unmarried until the end of the calendar year in which the child turns age twenty four. The dependent child of an employee shall not be eligible for fee payment benefits until the employee has completed one (1) continuous year of University employment.

Your spouse (legal or common-law, provided the common-law relationship has existed for one year or more) for health and dental insurance; Your unmarried children or your spouse's unmarried children (including adopted, xxxxxx or step-children) who are chiefly dependent on you for support and maintenance and for whom you are entitled to an income tax deduction, and provided they are up to 19 years of age. Under certain conditions as outlined below, coverage for dependent children may be extended: up to age 21 if qualified as a dependent according to the Federal Income Tax Act; or up to age 25 if qualified as a dependent and is a full time student at an accredited school, college or university; or up to age 25 if qualified as a dependent and is incapable of self-sustaining support or employment by reason of mental or physical disability.

20.03 Continuance of earnings as provided in 20.02 shall be subject to the submitting of acceptable evidence of its validity if requested by the Employer.
20.04 An employee shall cease to be covered by this plan on the first day of the month following the month in which he/she has not worked at least two (2) days as a result of layoff or Leave of Absence as per Article 10.00. The employee shall again be covered by this plan as soon as he/she returns to work from layoff or leave of absence. An employee on Leave of Absence may continue coverage during a Leave of Absence as provided he/she reimburses the Employer the full cost of the premium for such coverage for each month. An employee on layoff may continue coverage for a period of up to four (4) months provided he/she reimburses the Employer in advance the full cost of the premiums for such coverage for each month. Employees who work at least two (2) days in a month or fourteen (14) hours in a month shall be covered by this plan for that month.
