Plan Components. The Directed Growth Plan shall be completed using the Educator Plan Form in Appendix C-1. Each plan shall include, at a minimum, the following components: • The assigned primary evaluator and supervising evaluator, if any. • The start and end dates of the plan.
Plan Components i. The plan must cover more than the minimum for State re-licensure (six semester hours.)
ii. The plan can include professional growth in any or all of the six (6) areas listed in Policy 4880 section 5 “Activities for Horizontal Advancement.”

Plan Components. The Plan shall include medical coverage, including prescription drug coverage, and shall offer eligible employee's different options in choice of plans; dental insurance coverage; and vision insurance. The Board may offer these options through insured arrangements or on a self-insured basis through a third-party administrator. The Board shall select plan providers and third-party administrators through Board procurement processes.
Plan Components. The Employer will offer to each Employee a choice of two health insurance packages and optional benefits. • Package I is available to Employees at no cost. It will include, at minimum:
1. Medical Insurance coverage through Xxxxxx Permanente for Employee only.
2. Option to purchase Dependent coverage.
3. Choice of "Optional Benefits" described below.
1. Medial Insurance coverage for Employee only.
2. Dental and Vision coverage.
3. Group Life, Accidental Death and Dismemberment, and Long-Term Disability.
4. Option to purchase Dependent Medical coverage.
5. Option to purchase Additional Life Insurance.

Plan Components. The remediation plan shall specifically identify, in writing, the areas for improvement with targeted outcomes and/or activities that must be completed in order to address such areas.
Plan Components. The school professional growth plan shall be a one year plan with specific activities, goals, benchmarks, and outcomes, and will include appropriate professional development throughout the year needed to accomplish the goals and outcomes of the plan. LYHS Administration will attempt to provide sufficient professional development opportunities for teachers to obtain professional development credits to maintain their licenses, but each teacher will be personally responsible for meeting his/her professional development hours requirement.

iv. Beginning with school year 2010 – 2011, a teacher may only submit a Special Professional Growth Plan for approval on or before the following dates:
a. On or before September 1st, or
b. on or before February 1st.
v. The agreement on the plan must be reached between the principal (or designee) and the teacher before a teacher may begin work on the Special Professional Growth Plan.
vi. Once agreement is reached between the teacher and the principal (or designee) and Administrative Procedure 4141.3 has been completed it must be submitted to the Office of Human Resources.

Each plan shall include, at a minimum, the following components: The assigned Primary Evaluator, Supervising Evaluator (if any), and Contributing Evaluator (if any); The start and end date(s) of the plan;
