DHE Service Reporting. 16.2.1 Licensee shall provide to Licensor and its designee, if any, on a monthly basis (or, if possible, on a biweekly or weekly basis, and in any event on no less frequent basis than that provided to any other content provider), a statement in electronic form (“DHE Statement”) detailing the information specified by Licensor for each of the US DHE Service and the Canadian DHE Service from time to time including, but not limited to: (i) for each such DHE Included Program, the title of such program (including season and episode name, as applicable), and the total number of DHE Customer Transactions for each DHE Included Program made available by Licensor, (ii) with respect to such DHE Customer Transaction, the title of the DHE Included Program (including season and episode name, as applicable) and whether such DHE Included Program was exhibited in High Definition or Standard Definition), (iii) the DHE Customer Price applicable to each such DHE Customer Transaction and the type of currency in which the DHE Customer Price was billed to, and collected from, the DHE Customer with respect to such transaction, (iv) the DHE Customer Price for each DHE Included Program, and

DHE Service Reporting. 16.2.1 Licensee shall provide to Licensor and (subject to Section 16.5 below) its designee, if any, (a) on a monthly basis (or, if possible, on a biweekly or weekly basis, and in any event on no less frequent basis than that provided to any other provider of content on the Licensed Service) basis, a statement in electronic form (“DHE Statement”) emailed to Xxxx_xxxxxxx_xxxxxxx@xxx.xxxx.xxx detailing the information set forth in the example DHE Statement in Schedule H, (b) on a daily (but no less frequent basis than that provided to any other provider of content on the Licensed Service) basis, in electronic form via Licensee’s FTP site, the information set forth in the Schedule G, and (c) such other information about the DHE Included Programs that Licensor may reasonably request with no less than 30 days prior written notice and that Licensee, using commercially reasonable efforts, can provide. Licensee shall provide real-time DHE Statements to Licensor via Licensor’s FTP site if and when Licensee provides such reports in the same manner to any other Qualifying Studio. 16.2.2 If and when such information becomes available to Licensee, but in any event, if and when Licensee provides such information to any other provider of content on the Licensed Service, Licensee shall provide to Licensor and (subject to Section 16.5 below) its designee, if any, a statement in electronic form (“DHE Customer Statement”) emailed to Xxxx_xxxxxxx_xxxxxxx@xxx.xxxx.xxx detailing, on a DHE Customer-by-DHE Customer basis, the rights licensed to DHE Customer with respect to each DHE Included Program, including, without limitation (y) the then-current DHE Usage Rules associated with each DHE Included Program provided to the DHE Customer and (z) the entitlements DHE Customer has exercised with respect to such DHE Included Program (e.g., if the then-current DHE Usage Rules allow DHE Customer to download a copy that can be played on the PC, as well as a copy that can be played on a portable device, Licensee’s statements shall detail whether DHE Customer has or has not downloaded each such permitted copy); provided; however, that such statements shall not include any personally-identifiable DHE Customer information.
DHE Service Reporting. Licensee shall provide to Licensor and its designee, if any, statements containing periodic reports as set forth herein, in electronic form. Licensee shall provide monthly statements setting forth the following:
DHE Service Reporting. Licensee shall provide to Licensor and its designee, if any, on a weekly basis (or, if possible, on a daily basis, and in any event on no less frequent basis than that provided to any other content provider) basis, a statement in electronic form (“ DHE Statement”) emailed to Xxxx_xxxxxxx_xxxxxxx@xxx.xxxx.xxx detailing the information specified by Licensor for the DHE Service from time to time including, but not limited to the following, in each case separately for High Definition and Standard Definition and on a Territory-by-Territory basis: (i) the total number of DHE Customer Transactions for each DHE Included Program made available by Licensor; (ii) for each DHE Included Program, the number of DHE Customer Transactions per day during such month on the DHE Service, (iii) the DHE Customer Price applicable to each such DHE Customer Transaction; (iv) the DHE Customer Price for each DHE Included Program; (v) for each DHE Customer Transaction in such month, the currency in which such DHE Customer Transaction was billed and collected and reported; (vi) the total number of DHE Customer Transactions per DHE Included Program and for all DHE Included Programs supplied by Licensor, compared to the total number of user sessions on a monthly and cumulative basis; (vii) placement and promotion of the DHE Included Programs on the DHE Service; and (vii) such other information about the DHE Included Programs that Licensor may reasonably request with no less than 30 days prior written notice. Licensee shall provide real-time DHE Statements to Licensor if and when Licensee provides such reports to any other Qualifying Studio.

  • CHILD ABUSE REPORTING CONTRACTOR hereby agrees to annually train all staff members, including volunteers, so that they are familiar with and agree to adhere to its own child and dependent adult abuse reporting obligations and procedures as specified in California Penal Code section 11164 et seq. and Education Code 44691. To protect the privacy rights of all parties involved (i.e., reporter, child and alleged abuser), reports will remain confidential as required by law and professional ethical mandates. A written statement acknowledging the legal requirements of such reporting and verification of staff adherence to such reporting shall be submitted to the LEA.

  • Performance Reporting The State of California is required to submit the following financial reports to FEMA:

  • Monthly Reporting Within twenty (20) calendar days following the end of each calendar month, Registry Operator shall deliver to ICANN reports in the format set forth in Specification 3 attached hereto (“Specification 3”).

  • Research Use Reporting To assure adherence to NIH GDS Policy, the PI agrees to provide annual Progress Updates as part of the annual Project Renewal or Project Close-out processes, prior to the expiration of the one (1) year data access period. The PI who is seeking Renewal or Close-out of a project agree to complete the appropriate online forms and provide specific information such as how the data have been used, including publications or presentations that resulted from the use of the requested dataset(s), a summary of any plans for future research use (if the PI is seeking renewal), any violations of the terms of access described within this Agreement and the implemented remediation, and information on any downstream intellectual property generated from the data. The PI also may include general comments regarding suggestions for improving the data access process in general. Information provided in the progress updates helps NIH evaluate program activities and may be considered by the NIH GDS governance committees as part of NIH’s effort to provide ongoing stewardship of data sharing activities subject to the NIH GDS Policy.

  • Compliance Reporting a. Provide reports to the Securities and Exchange Commission, the National Association of Securities Dealers and the States in which the Fund is registered. b. Prepare and distribute appropriate Internal Revenue Service forms for corresponding Fund and shareholder income and capital gains. c. Issue tax withholding reports to the Internal Revenue Service.

  • Quarterly Contractor Performance Reporting Customers shall complete a Contractor Performance Survey (Exhibit I) for each Contractor on a Quarterly basis. Customers will electronically submit the completed Contractor Performance Survey(s) to the Department Contract Manager no later than the due date indicated in Contract Exhibit D, Section 17, Additional Special Contract Conditions. The completed Contractor Performance Survey(s) will be used by the Department as a performance-reporting tool to measure the performance of Contractors. The Department reserves the right to modify the Contractor Performance Survey document and introduce additional performance-reporting tools as they are developed, including online tools (e.g. tools within MyFloridaMarketPlace or on the Department's website).

  • Sales Reporting Requirements This is a requirement of the TIPS Contract and is non-negotiable. By submitting this proposal, Vendor certifies that Vendor will properly report all TIPS sales. With the exception of TIPS Automated Vendors, who have signed an exclusive agreement with TIPS regarding reporting, all TIPS Sales must be reported to TIPS by either: (1) Emailing the purchase order or similar purchase document (with Vendor’s Name, as known to TIPS, and the TIPS Contract Name and Number included) to TIPS at xxxxxx@xxxx-xxx.xxx with “Confirmation Only” in the subject line of the email within three business days of Vendor’s acceptance of the order, or; (2) Within 3 business days of the order being accepted by Vendor, Vendor must login to the TIPS Vendor Portal and successfully self-report all necessary sale information within the Vendor Portal and confirm that it shows up accurately on your current Vendor Portal statement. No other method of reporting is acceptable unless agreed to by the Parties in writing. Failure to report all sales pursuant to this provision may result in immediate cancellation of Vendor’s TIPS Contract(s) for cause at TIPS’ sole discretion.

  • CONTRACT SALES REPORTING Contractor shall report total contract sales quarterly to Enterprise Services, as set forth below.

  • Loop Testing/Trouble Reporting 2.1.6.1 Telepak Networks will be responsible for testing and isolating troubles on the Loops. Telepak Networks must test and isolate trouble to the BellSouth portion of a designed/non-designed unbundled Loop (e.g., UVL-SL2, UCL-D, UVL-SL1, UCL-ND, etc.) before reporting repair to the UNE Customer Wholesale Interconnection Network Services (CWINS) Center. Upon request from BellSouth at the time of the trouble report, Telepak Networks will be required to provide the results of the Telepak Networks test which indicate a problem on the BellSouth provided Loop. 2.1.6.2 Once Telepak Networks has isolated a trouble to the BellSouth provided Loop, and had issued a trouble report to BellSouth on the Loop, BellSouth will take the actions necessary to repair the Loop if a trouble actually exists. BellSouth will repair these Loops in the same time frames that BellSouth repairs similarly situated Loops to its End Users. 2.1.6.3 If Telepak Networks reports a trouble on a non-designed or designed Loop and no trouble actually exists, BellSouth will charge Telepak Networks for any dispatching and testing (both inside and outside the CO) required by BellSouth in order to confirm the Loop’s working status. 2.1.6.4 In the event BellSouth must dispatch to the end-user’s location more than once due to incorrect or incomplete information provided by Telepak Networks (e.g., incomplete address, incorrect contact name/number, etc.), BellSouth will xxxx Xxxxxxx Networks for each additional dispatch required to repair the circuit due to the incorrect/incomplete information provided. BellSouth will assess the applicable Trouble Determination rates from BellSouth’s FCC or state tariffs.

  • Diversity Reporting Upon request, the Contractor will report to the Department its spend with business enterprises certified by the OSD. These reports must include the time period covered, the name and Federal Employer Identification Number of each business enterprise utilized during the period, commodities and contractual services provided by the business enterprise, and the amount paid to the business enterprise on behalf of each agency purchasing under the Contract.
