Employment Screening. 4.14.1 The Provider shall ensure that all staff utilized by the Provider and its subcontractors (hereinafter, “Contracted Staff”) that are required by Florida law and by CFOP 60-25, Chapter 2, which is hereby incorporated by reference to be screened in accordance with chapter 435, F.S., are of good moral character and meet the Level 2 Employment Screening standards specified by sections 435.04, 110.1127, and subsection 39.001(2), F.S., as a condition of initial and continued employment that shall include but not be limited to: 4.14.1.1 Employment history checks; 4.14.1.2 Fingerprinting for all criminal record checks; 4.14.1.3 Statewide criminal and juvenile delinquency records checks through the Florida Department of Law Enforcement (FDLE); 4.14.1.4 Federal criminal records checks from the Federal Bureau of Investigation via the Florida Department of Law Enforcement; and 4.14.1.5 Security background investigation, which may include local criminal record checks through local law enforcement agencies. 4.14.1.6 Attestation by each employee, subject to penalty of perjury, to meeting the requirements for qualifying for employment pursuant to chapter 435 and agreeing to inform the employer immediately if arrested for any of the disqualifying offenses while employed by the employer. 4.14.2 The Provider shall sign the Florida Department of Children and Families Employment Screening Affidavit each State fiscal year (no two such affidavits shall be signed more than 13 months apart) for the term of the Contract stating that all required staff have been screened or the Provider is awaiting the results of screening. 4.14.3 The Department requires, as applicable, the use of the Officer of Inspector General’s Request for Reference Check form (CF 774), which states: “As part of the screening of an applicant being considered for appointment to a career service, selected exempt service, senior management, or OPS position with the Department of Children and Families or a Contract or sub-contract provider, a check with the Office of Inspector General (IG) is required to determine if the individual is or has been a subject of an investigation with the IG’s Office. The request will only be made on the individual that is being recommended to be hired for the position if that individual has previously worked for the Contract or sub-contract provider, or if that individual is being promoted, transferred or demoted within the Contract or sub-contract provider.”
Employment Screening. Executive has successfully completed a pre-employment consumer report verification, and Employer new hire paperwork, each of which was to be conducted in accordance with applicable state and/or federal law. Executive understands and agrees that Executive will be subject to Employer’s general policies and practices concerning applicants for senior executive positions and new senior executive employees.

Stat., are of good moral character and meet the Level 2 Employment screening standards specified in § 394.4572, 397.451, 408.809, 435.04, 110.1127 and §39.001(2), Fla. Stat., as a condition of initial and continued employment that shall include, but not be limited to: i. Employment history check; ii. Fingerprinting for all criminal record checks; iii. Statewide criminal and juvenile delinquency records checks through the Florida Department of Law Enforcement (FDLE): iv. Federal criminal records checks from the Federal Bureau of Investigations via the FDLE; and v. Security background investigation, which may include local criminal records checks through local law enforcement agencies. vi. Attestation by each employee, subject to penalty, to meeting the requirements for qualifying for employment pursuant to chapter 435 and agreeing to inform the employer immediately if arrested for any of the disqualifying offenses while employed by the employer. Employment Screening. In balancing the NJIT community’s right to have a drug- free Public Safety Department against the Public Safety Officer’s right to privacy, urinalysis will be the chosen method of screening. Screening will be administered as follows: (1) screening of all Public Safety Officers, in the absence of “reasonable individualized suspicion” may be conducted at NJIT’s discretion at any time upon five (5) calendar days’ notice or (2) individual screening will be administered only when there is a “reasonable individualized suspicion” to believe that an individual Public Safety Officer is using drugs in violation of policy. Testing will be required based on the objective facts and on rational inferences, drawn by a supervisor, to indicate that the usage of any drug may be the cause of an unfit condition where the Public Safety Officer is: a. Impaired in, or incapable of performing his or her assigned duties, and/or b. The subject of investigation “where sufficient evidence and facts are present to constitute a reasonable/individualized suspicion” that he or she is involved in the use, possession or sale of drugs and/or c. Experiencing (without plausible, legitimate explanation proffered and fully validated) excessive absenteeism and significantly reduced productivity, and/or d. Found to have excessive or otherwise suspicious on-duty injuries or other accidents indicating negligence in relation to assigned duties; and/or e. Exhibiting unusual and/or bizarre behavior patterns, reasonably deemed inconsistent with regular and appropriate behavior patterns.
