EXHIBIT A PROJECT DESCRIPTION. The Certified Local Government (CLG) matching grants program provides funding for participating city and county governments in Illinois to develop and sustain effective local preservation programs and projects that are critical to preserving local historic resources. The Illinois Department of Natural Resources (IDNR) is required to subgrant at least ten percent (10%) of Illinois’ annual federal allocation of National Park Service (NPS) Historic Preservation Fund (HPF) to participating CLGs. Currently, there are 81 CLGs in Illinois. Contingent on resolution of the federal budget for its fiscal year 2021 and finalization of a federal award, IDNR anticipates that approximately $140,340 will be available – on a competitive basis – for this round of grants. All grant proposals submitted should reflect a minimum of 30% matching funds. State Historic Preservation Office (SHPO; federal) Share = 70%; Matching (local) Share = 30%. Funded project types include the following: • Survey Projects; • National Register Nomination and Listing Revision Projects; • Planning Projects; • Public Education Projects, including Materials Publication. Applicants must develop projects in accordance with the Secretary of Interior’s Standard for Historic Preservation and must identify a qualified project manager who meets the professional qualification standards in 36 CFR Part 61. CLG grants are reimbursement grants, requiring grantees to expend local funds and submit documentation for reimbursement. The Xxxxxxx County Historic Preservation Commission seeks to conduct an intensive-level historical and architectural survey of Xxxxxxx and Bristol Townships, two of the nine townships within Xxxxxxx County. By ordinance, the Commission has purview over the unincorporated portions of the county. Therefore, a survey of the incorporated City of Yorkville is not included. The work of this survey project will be divided between an outside consulting firm engaged by the county and volunteers recruited by the commission. The survey project will be led and organized by Xxxxxxx Xxxx of Wiss, Xxxxxx, Xxxxxxx Associates. The field survey work will be conducted by two-person teams, typically one WJE professional architectural history staff person and one volunteer. Historical research will mainly be conducted by volunteers. The data compilation and report writing will be performed by WJE. EXHIBIT A PROJECT DESCRIPTION. The City of Lake Forest (“City”) has allocated $15,000 for a Contractor to operate an Animal Adoption Center within the city. As part of this endeavor, the City has partnered with Orange County Animal Care which is providing an additional $15,000 for this pilot program, increasing the total amount of available funding to $30,000. The goal of the pilot program is to reduce shelter euthanasia for dogs and cats; and, promote shelter adoption, microchipping, and responsible pet ownership. It is the intent of the Parties that the animals housed and offered for adoption by the Project consist primarily of animals made available by OC Animal Care (“OCAC”) to Contractor for adoption by Contractor (“Provided Shelter Animals”). To that end, City will ensure that at least sixty percent (60%) of the Project’s available space and resources be allocated, at all times, to Provided Shelter Animals (the “Allocation Amount”). To ensure that the Project achieves the Parties’ goal of reducing shelter euthanasia, it is the intent of the Parties that the Provided Shelter Animals consist of a wide variety of animals including animals at risk for euthanasia. Therefore, the Provided Shelter Animals made available to the Project will be chosen solely at the discretion of OCAC and will include animals at risk for euthanasia. City will ensure that Contractor adopts sufficient Provided Shelter Animals proffered by OCAC to satisfy the Allocation Amount. OCAC will only proffer as Provided Shelter Animals dogs, cats and rabbits (the “Common Species”) though Contractor may elect to adopt animals of other species at its option. Additional animals, whether of the Common Species or another species, chosen by Contractor and not identified by OCAC as Provided Shelter Animals will not count toward the 60% allocation reserved for Provided Shelter Animals. Provided Shelter Animals will be drawn from all areas served by OCAC and will not be limited to animals originating from Lake Forest. The Parties acknowledge that animals received by the Project from owners residing in the jurisdiction that OCAC serves, that would have been impounded by OCAC upon owner surrender, and with respect to whom the previous owner expressed an intention to surrender to County but, due to intervention efforts, were redirected to the Project, shall count as Provided Shelter Animals. EXHIBIT A PROJECT DESCRIPTION. The XxXxxx County ARI program is a program of the Intensive Probation Services Unit to provide treatment and ancillary services and enhanced supervision to all moderate to high risk/need offenders eligible according to the Crime Reduction Act that are not being served in other XxXxxx County probation programs. The program utilizes an Intensive Probation Supervision with Services model. The evidence-based practices used include random drug testing, risk/need assessment with a validated instrument, case planning to match risk/need, motivational interviewing, cognitive behavioral therapy, clinical and trauma evaluation and services, graduated sanctions and incentives, moral reconation therapy, medication assisted treatment, and Xxxxx guides. XxXxxx XXX will take any felony offender that is not excluded by the violent offender clause of the Crime Reduction Act (Class 1-4 Felonies). There was an average of 112 ARI eligible offenders sentenced to prison from XxXxxx County between SFY15- SFY17, thereby requiring that 28 people be diverted in SFY19 to meet the 25% reduction target. XxXxxx County seeks support from Adult Redeploy Illinois to provide a continuum of local, community-based sanctions and treatment alternatives for non-violent offenders who would otherwise be incarcerated if those local services and sanctions were not available. STATEMENT OF PROBLEM Every year, approximately 13,000 non-violent offenders are sent to the Illinois Department of Corrections, including 112 from XxXxxx County (2015-2017 average). While research shows that most non-violent offenders are supervised in the community with better outcomes and at lower cost, local resources for intensive supervision and treatment services are insufficient to meet offenders’ needs and public safety concerns. Since 2009, the number of felony sentences to probation in XxXxxx County has increased 55%, and, according to a study done by Dr. Xxxxx Xxxxx of Loyola University, XxXxxx County is using prison in only 28% of felony convictions. This means 72% of convicted felons are staying in the community on community supervision. This has created a caseload burden, leaving many officers unable to give the individual time and attention higher risk/need offenders require for a rehabilitative effect. ARI is able to offer additional stabilization services for eligible clients that may otherwise be dismissed with PTRs. The Grantee will implement objectives, strategies and activities to achieve policy, systems and environmental changes which adhere to the federal Centers for Disease Control and Prevention (CDC), Office on Smoking and Health, Best Practices for Comprehensive Tobacco Control Programs (2014) and other national guidelines for tobacco prevention and control activities. The Danish AD Association (AEF) wishes to launch a disease awareness campaign on SoMe empowering patients to revisit the health care system an require optimal treatment and disease control. The association plan to reuse and adjust existing empowerment videos where AD-patients, caregivers and dermatologists reach out to patients and deliver hope and guidance on how and where to get adequate help.
