EXHIBIT A PROJECT DESCRIPTION. The City of Lake Forest (“City”) has allocated $15,000 for a Contractor to operate an Animal Adoption Center within the city. As part of this endeavor, the City has partnered with Orange County Animal Care which is providing an additional $15,000 for this pilot program, increasing the total amount of available funding to $30,000. The goal of the pilot program is to reduce shelter euthanasia for dogs and cats; and, promote shelter adoption, microchipping, and responsible pet ownership.

EXHIBIT A PROJECT DESCRIPTION. The XxXxxx County ARI program is a program of the Intensive Probation Services Unit to provide treatment and ancillary services and enhanced supervision to all moderate to high risk/need offenders eligible according to the Crime Reduction Act that are not being served in other XxXxxx County probation programs. The program utilizes an Intensive Probation Supervision with Services model. The evidence-based practices used include random drug testing, risk/need assessment with a validated instrument, case planning to match risk/need, motivational interviewing, cognitive behavioral therapy, clinical and trauma evaluation and services, graduated sanctions and incentives, moral reconation therapy, medication assisted treatment, and Xxxxx guides. XxXxxx XXX will take any felony offender that is not excluded by the violent offender clause of the Crime Reduction Act (Class 1-4 Felonies). There was an average of 112 ARI eligible offenders sentenced to prison from XxXxxx County between SFY15- SFY17, thereby requiring that 28 people be diverted in SFY19 to meet the 25% reduction target. XxXxxx County seeks support from Adult Redeploy Illinois to provide a continuum of local, community-based sanctions and treatment alternatives for non-violent offenders who would otherwise be incarcerated if those local services and sanctions were not available. Every year, approximately 13,000 non-violent offenders are sent to the Illinois Department of Corrections, including 112 from XxXxxx County (2015-2017 average). While research shows that most non-violent offenders are supervised in the community with better outcomes and at lower cost, local resources for intensive supervision and treatment services are insufficient to meet offenders’ needs and public safety concerns. Since 2009, the number of felony sentences to probation in XxXxxx County has increased 55%, and, according to a study done by Dr. Xxxxx Xxxxx of Loyola University, XxXxxx County is using prison in only 28% of felony convictions. This means 72% of convicted felons are staying in the community on community supervision. This has created a caseload burden, leaving many officers unable to give the individual time and attention higher risk/need offenders require for a rehabilitative effect. ARI is able to offer additional stabilization services for eligible clients that may otherwise be dismissed with PTRs. The Grantee will provide, through implementation of its Illinois Tobacco-Free Communities (ITFC) work plan, a local tobacco prevention and control program implementing science-based, policy-focused strategies aligned with State and national goals and objectives to prevent tobacco use among youth, promote tobacco use cessation among adults and youth, eliminate exposure to secondhand smoke, enforce the Smoke-Free Illinois Act and local tobacco control ordinances, and identify and eliminate tobacco-related health disparities among specific population groups. The Grantee will implement objectives, strategies and activities to achieve policy, systems and environmental changes which adhere to the federal Centers for Disease Control and Prevention (CDC), Office on Smoking and Health, Best Practices for Comprehensive Tobacco Control Programs (2014) and other national guidelines for tobacco prevention and control activities. EXHIBIT A PROJECT DESCRIPTION. The Certified Local Government (CLG) matching grants program provides funding for participating city and county governments in Illinois to develop and sustain effective local preservation programs and projects that are critical to preserving local historic resources. The Illinois Department of Natural Resources (IDNR) is required to subgrant at least ten percent (10%) of Illinois’ annual federal allocation of National Park Service (NPS) Historic Preservation Fund (HPF) to participating CLGs. Currently, there are 81 CLGs in Illinois. Contingent on resolution of the federal budget for its fiscal year 2021 and finalization of a federal award, IDNR anticipates that approximately $140,340 will be available – on a competitive basis – for this round of grants.   • System Description The wet detention basin is designed to trap 80% of sediment in runoff and maintain pre-development downstream peak flows. The basin has two forebays (smaller ponds) located at the low end of two grass xxxxxx. In addition to runoff conveyance, the grass xxxxxx also allow infiltration and filtering of pollutants, especially from smaller storms. The forebays are each 4 feet deep. They are connected to the main pool by 18 and 24-inch metal pipes that outlet onto a rock chute. The forebays will trap coarse sediments in runoff, such as road sands, thus reducing maintenance of the main basin. The main pool will trap the finer suspended sediment. To do this, the pond size, water level and outlet structures must be maintained as specified in this Agreement (see Figures 1, 2 and 3). The main basin receives runoff from a 67.1 acre drainage area (41.2 acres within the subdivision and 25.9 acres off-site drainage coming from the east). During high rainfall or snow melt events, the water level will temporarily rise and slowly drain down to the elevation of the control structure. The water level is controlled by a 12-inch concrete pipe extending through the berm in the northwest corner of the basin (see Figures 1 and 3). On the face of the 12-inch pipe, there is metal plate with a 3-inch drilled hole (orifice) with stone in front of it. This orifice controls the water level and causes the pond to temporarily rise during runoff events. Washed stone (1- 2” diameter) is placed in front of the orifice to prevent clogging. High flows may enter the grated concrete riser or flow over the rock lined emergency spillway. “As-built” construction drawings of the basin, showing actual dimensions, elevations, outlet structures, etc. will be recorded as an addendum(s) to this agreement within 60 days after [Municipality Name] accepts verification of construction from the project engineer.
