Collaborations‌‌ Applicants must describe how they will collaborate with programs and organizations either internal or external to ASPR and CDC. Strategies and Activities. The goal of the PHEP program is to develop effective public health emergency management and response programs nationwide. By the end of the performance period, PHEP recipients should build and sustain the public health emergency preparedness and response capabilities to achieve substantial, measurable progress across the six domains. Additionally, PHEP recipients are expected to demonstrate operational readiness across all domains by the end of the performance period. CDC has developed supplemental resources to assist PHEP recipients in developing work plans that address the strategies and activities outlined in the PHEP logic model. The 2019-2024 PHEP Supplemental Guidance and Resources are available at xxxxx://xxx.xxx.xxx/cpr/readiness/phep For the 2019-2024 performance period, all PHEP recipients must address and comply with the following programmatic requirements for the PHEP logic model strategies and activities, building jurisdictional capability with the goal of achieving operational readiness for emerging infectious diseases (EIDs) such as pandemic influenza, an anthrax event, and other public health threats and hazards. In addition, PHEP recipients must meet all statutory requirements and HHS grant guidance requirements. CDC has modified programmatic requirements for U.S.-Affiliated Pacific Islands (USAPI) and U.S. Virgin Islands (USVI) recipients. More information is available in the 2019-2024 PHEP Supplemental Guidance and Resources: Modified Requirements for USAPI and USVI Recipients. CDC does not have modified requirements for Puerto Rico. For the purposes of this NOFO, Puerto Rico must meet the same requirements as the 50 states. By the end of the performance period, PHEP recipients are expected to show measurable progress toward meeting the outcomes in the PHEP logic model by implementing the following strategies and activities organized by capability domains. CDC will require evidence of domain strategies and activities in various ways, such as work plans and other application documentation; exercise and training plans; annual progress reports; quarterly action plans; XXX documentation; and after-action reports and improvement plans (AARs/IPs).
Strategies and Activities. Each TB program funded under this NOFO will be categorized according to one of the following definitions: • High-incidence jurisdiction: Program reporting ≥150 TB cases annually, or • Low-incidence jurisdiction: Program reporting < 150 TB cases annually. To accomplish the priority activity of identifying individuals with suspected or confirmed TB disease and ensure standard and appropriate treatment regimens, the following should be conducted: • Ensure case management and treatment of persons with active TB through the use of adherence-promoting measures such as case review/cohort analysis, outreach staff who are culturally competent, extensive application of conventional and electronic directly observed therapy (DOT, and eDOT), incentives, and enablers. • Assess adequacy and appropriateness of therapy for each patient by reviewing initial regimen, susceptibility results, adherence, and response to therapy. Therapy should be consistent with American Thoracic Society/ Infectious Disease Society of America/Centers for Disease Control and Prevention guidelines. CDC’s TB Centers of Excellence (XXX) for Training, Education, and Medical Consultation services should be readily used as needed, and relationships should be fostered between any local TB experts who provide the program with clinical guidance and the funded jurisdictions' regional XXX. Healthcare providers of record for patients with TB disease should be expected to be familiar with laboratory and chest imaging results as well as current medications and comorbidities when seeking additional medical consultation services. • Seek expert consultation regarding laboratory results for molecular detection of drug resistance or interpretation of other laboratory results when needed. To achieve this goal, the PHEP strategies and activities focus on 1) areas for which improvement has been identified in drills, exercises, and incident responses across each of the public health preparedness capabilities and medical countermeasure (MCM) technical assistance action plans and 2) program requirements for the project period. For the 2017-2022 project period, HPP and PHEP awardees must address and comply with joint, HPP- specific, and PHEP-specific programmatic requirements for the strategies and activities listed below, as well as other requirements associated with statute and HHS grant guidance. In completing the program requirements segment of the funding application, awardees must provide updates on joint, HPP- specific, and PHEP- specific requirements. Strategies and Activities. The strategies to be utilized in realizing the goals and objectives of the MARC project are informed by the RWJF Roadmap to Resilience as well as by the FSG’s Five Conditions for Collective Success. Though we intend to apply these strategies to achieve our short term goals, we recognize that they are just the first steps toward achieving the larger objectives of increasing understanding of the impact of trauma and adversity on neurodevelopment, protecting youth from the impact of toxic stress, improving surveillance so that affected children are identified early in life, increasing our capacity to help affected children heal and reducing our tendency to reinjure people who have already been adversely impacted. Sonoma County is fortunate to have a long history of successful community partnerships among multiple disciplines and providers, and has developed a commitment to and effectiveness with community engagement and collective impact. The SCAC has already been able to implement some of the strategies illustrated in the Roadmap.
