Exit Interviews. A. If a regular status employee provides timely notice that they are voluntarily separating from Agency service, the Agency will offer an exit interview that focuses on the reason(s) for the employee leaving Agency service and what changes they recommend to the Agency to improve Agency operations, or,
B. A Department of Administrative Services written instrument.
C. Upon request, but no more than two (2) times a year, the Union can receive a report of the Department of Administrative Services written instrument results from employee feedback on their Agency experience.
Exit Interviews. Any employee terminating active employment with the City will be given an exit interview by an employee of the Human Resources Department and/or Finance Department during which time an explanation of all rights and benefits, as well as an accounting of all sums due, will be provided to the employee or, in the case of death, this information will be provided to his or her spouse.

Upon request a nurse shall be granted an exit interview conducted by the Human Resources department or another leader as designated by Human Resources when transferring to a different program or upon discharge/resignation of employment. A summarized copy of the interview will be provided to the Association upon authorization of the exiting nurse.

Nurses changing departments may request an exit interview (which may be in survey form). For exit interviews given in survey form, the survey will include an option, at the top of the survey, for the employee to opt-in to having their responses anonymously shared with the Association. Employees may request a copy of their exit interview responses. The Employer shall share the survey responses of those bargaining unit employees who opted in with the Association on a quarterly basis.
Exit Interviews. 1. During the last month of employment, the Superintendent of Schools will conduct an exit interview with the terminating employee. The purpose of the interview is to receive constructive criticism from the employee and to reaffirm the reasons for termination. (Adopted 2/16/81)
Exit Interviews. Upon an employee’s resignation or retirement, he shall be afforded the opportunity for an interview with the 911 Director and Sheriff. The purpose of this interview shall be to allow the employee to express reasons for termination and his/her views on the operation of the Sheriff’s Office.
