Illness on Long Service Leave. 27.5.1 Subject to the production of a supporting medical certificate, a teacher who becomes ill whilst on long service leave is entitled to have the period of illness treated as sick leave but only to the extent that the teacher is entitled to sick leave.
Illness on Long Service Leave. Subject to the requirements of 28.12, an Employee, who becomes ill or suffers an injury during long service leave and has an entitlement to sick leave, may apply to the Employer for the period of illness or injury to be treated as sick leave, with long service leave reaccredited to the Employee. The Employer may require the Employee to be examined by a registered medical practitioner of the Employer’s choice, provided the practitioner is reasonably accessible to the Employee.
Illness on Long Service Leave. (a) Subject to the production of a medical certificate from a Medical Practitioner, if an Employee is ill or injured whilst on long service leave the Employee is entitled to have the period of illness/injury treated as personal leave but only to the extent that the Employee is entitled to personal leave.
(b) Subject to cl.18.5(a), the Employee’s long service leave will be extended by the period of illness or injury.
(c) An exception to cl.18.5(b) is that an Employer and an Employee may agree that the Employee will return from long service leave as planned with the period of illness or injury increasing the teacher’s accrued long service leave entitlement.
Illness on Long Service Leave. 52.2.1. Subject to the requirements of cl. 52.2.2, an Employee, who becomes ill or suffers an injury during long service leave and has an entitlement to sick leave, is entitled to have the period of illness or injury treated as sick leave, with long service leave reaccredited to the Employee. The Principal may require the Employee to be examined by a registered medical practitioner of the Employer’s choice, provided the practitioner is reasonably accessible to the Employee.
52.2.2. The Employee’s application under cl. 52.2.1: must be received by the Employer during the period of illness or injury;
i) must be accompanied by a medical certificate from a Registered Health Practitioner or a statutory declaration attesting to the illness or injury and the duration of that illness or injury; and,
ii) must indicate whether the Employee wishes to extend the long service leave by the period of the illness or injury or whether the Employee will return from long service leave as planned with the period of illness or injury increasing the Employee’s accrued long service leave entitlement.
Illness on Long Service Leave. (a) Subject to the requirements of cl.22.8(b), a Staff Member, who becomes ill or suffers an injury during long service leave and has an entitlement to personal/carer’s leave, is entitled to have the period of illness or injury treated as personal/carer’s leave, with long service leave reaccredited to the Staff Member.
(b) The Staff Member’s application under cl.22.8(a):
(i) must be received by Woodleigh School during the period of illness or injury; and
(ii) must be accompanied by a medical certificate from a Registered Medical Practitioner or a statutory declaration attesting to the illness or injury and the duration of that illness or injury.
Illness on Long Service Leave. 18.2.1 Subject to the requirements of 18.4, an Employee, who becomes ill or suffers an injury during long service leave and has an entitlement to sick leave, is entitled to have the period of illness or injury to be treated as sick leave, with long service leave reaccredited to the Employee. The Principal may require the Employee to be examined by a registered medical practitioner of the Employer's choice, provided the practitioner is reasonably accessible to the Employee.
18.2.2 The Employee's application;
i. must be received by the Employer during the period of illness or injury;
ii. must be in writing unless the injury or illness is such that the Employee is unable to write.
iii. must be accompanied by a medical certificate from a registered health practitioner or a statutory declaration attesting to the illness or injury and the duration of that illness or injury; and
iv. must indicate whether the Employee wishes to extend the long service leave by the period of the illness or injury or whether the Employee will return from long service leave as planned with the period of illness or injury increasing the Employee's accrued long service leave entitlement.
Illness on Long Service Leave. 41.3.1. Subject to the requirements of cl. 41.3.2, a Teacher who becomes ill or suffers an injury during long service leave and has an entitlement to sick leave, is entitled to have the period of illness or injury treated as sick leave, with long service leave reaccredited to the Teacher. The Principal may require the Teacher to be examined by a registered medical practitioner of the Teacher’s choice, provided the practitioner is reasonably accessible to the Teacher.
41.3.2. The Teacher’s application under cl. 41.3.1: must be received by the Employer during the period of illness or injury;
i) must be accompanied by a medical certificate from a Registered Health Practitioner or a statutory declaration attesting to the illness or injury and the duration of that illness or injury; and, must indicate whether the Teacher wishes to extend the long service leave by the period of the illness or injury or whether the Teacher will return from long service leave as planned with the period of illness or injury increasing the Teacher’s accrued long service leave entitlement.
41.3.3. A Teacher who has completed seven years’ continuous employment may apply for pro-rata long service leave.

(c) An exception to 11.4.2 is that an employer and an employee may agree that the employee will return from long service leave as planned with the period of illness increasing the employee's accrued long service leave entitlement.
Illness on Long Service Leave. The Principal may require the Employee to be examined by a registered medical practitioner of the Employer’s choice, provided the practitioner is reasonably accessible to the Employee.
(b) The Employee’s application under cl.26.1.3(a):
(i) must be received by the Employer during the period of illness or injury;
(ii) must be accompanied by a medical certificate from a Registered Medical Practitioner or a statutory declaration attesting to the illness or injury and the duration of that illness or injury; and
(iii) must indicate whether the Employee wishes to extend the long service leave by the period of the illness or injury or whether the Employee will return from long service leave as planned with the period of illness or injury increasing the Employee’s accrued long service leave entitlement.
Illness on Long Service Leave. 19.2.1 Subject to 19.4, an Employee, who becomes ill or suffers an injury during long service leave and has an entitlement to sick leave, may apply to the Principal for the period of illness or injury to be treated as sick leave, with long service leave reaccredited to the Employee. An application must be made pursuant to 19.4. The Principal may require the Employee to be examined by a registered medical practitioner of the Employer’s choice, provided the practitioner is reasonably accessible to the Employee and may be granted at the Principal’s discretion based on the evidence provided.
19.2.2 The Employee’s application:
(i) must be in writing and received by the Employer during the period of illness or injury when the Employee is physically capable of so doing;
(ii) must be accompanied by a medical certificate from a registered health practitioner or a statutory declaration attesting to the illness or injury and the duration of that illness or injury; and
(iii) must indicate whether the Employee wishes to extend the long service leave by the period of the illness or injury or whether the Employee will return from long service leave as planned with the period of illness or injury increasing the Employee’s accrued long service leave entitlement.
