Job Incurred Disability Leave. Job incurred disability leave with pay is an employee's absence from duty with pay because of disability caused by illness or injury arising out of and in the course of his/her employment which has been declared to be compensable under the Workers' Compensation Law. Only permanent or probationary employees occ upying permanent positions are eligible for job incurred disability leave with pay. Payment of job incurred disability leave shall be at the base pay of the employee, and shall be reduced by the amount of temporary disability indemnity received, pursuant to Workers' Compensation Law. In order to receive pay for job incurred disability leave an employee must submit a request on a prescribed form to the City Manager describing the illness or accident and all information required for the City Manager to evaluate the request. The employee must attach to the request a statement from a physician certifying to the nature, extent, and probable period of illness or disability. No job incurred disability leave with pay may be granted until after the City has decla red the illness or injury to be compensable under the California Workers' Compensation Law and has accepted liability. Eligible employees shall be entitled to disability leave for a period of incapacity as determined by a physician, but not to exceed a maximum of thirty (30) calendar days for any one illness or injury. After the thirty (30) calendar day period if still injured, the employee is entitled to temporary disability payments as defined by the laws governing workers’ compensation.

  • Pregnancy Disability Leave A. Leave for pregnancy or childbirth related disability is in addition to any leave granted under FMLA or WFLA.

  • Disability Leave a. An employee who has suffered possible injury in the performance of assigned duties shall immediately undergo such medical examination as the appointing authority deems necessary. He/she shall not be considered absent from duty during the time required for such examination.

  • Short-Term Disability Leave In order to access short-term disability leave, medical confirmation may be requested and shall be provided on the form attached as Appendix “C” to this Agreement. In either instance where an Employee does not provide medical confirmation as requested, or otherwise declines to participate and/or cooperate in the administration of the Sick Leave Plan, access to compensation may be suspended or denied. Before access to compensation is denied, discussion will occur between the union and the school board. Compensation will not be denied for the sole reason that the medical practitioner refuses to provide the required medical information. A school board may require an independent medical examination to be completed by a medical practitioner qualified in respect of the illness or injury of the Board’s choice at the Board’s expense. In cases where the Employee’s failure to cooperate is the result of a medical condition, the Board shall consider those extenuating circumstances in arriving at a decision.

  • Temporary Disability Leave A temporary disability leave of absence shall be granted to teachers of this school corporation on the following basis:

  • Permanent Disability Permanent Disability" shall mean Employee's physical or mental incapacity to perform his or her usual duties with such condition likely to remain continuously and permanently as determined by Employer.
