Long Term Disability Insurance Benefits. A Member who remains totally disabled after the expiry of seventeen (17) weeks of continuous disability shall become entitled to claim benefits under the Long Term Disability Insurance Plan subject to the conditions of that Plan.

Long Term Disability Insurance Benefits. A-5.01 Subject to the rules and regulations of the plan, the Employer shall provide all eligible full-time employees with Long Term Disability Insurance Benefits through its group policy. The Employer pays sixty (60%) percent of the premium cost and the full-time employee pays forty (40%) percent of the premium cost.
Long Term Disability Insurance Benefits. The District shall provide each eligible supervisor with long-term disability insurance as follows:
15.5.1 The insurance shall provide a disability payment equal to 66-2/3% of the supervisor's “basic monthly earnings” on the date he or she was disabled to a maximum payment of $6,000 per month. “Basic monthly earnings” means 1/12th of the supervisor's annual contract salary.

13.2 The coverage for the plan is as follows:

22.6.1 For employees disabled on or after March 1, 1999 through December 31, 2007 the insurance shall provide a benefit equal to 66 2/3 percent of the employee’s “Basic monthly earnings” on the date he or she became disabled, to a maximum payment of $4,300 per month. “Basic monthly earnings” means 1/12th of the employee’s annual contract salary.
22.6.2 For employees disabled on or after January 1, 2008 the insurance shall provide a benefit equal to 66 2/3 percent of the employee’s “Basic monthly earnings” on the date he or she became disabled, to a maximum payment of $6,000 per month. “Basic monthly earnings” means 1/12 of the employee’s annual contract salary.
22.6.3 The benefits shall be payable after the end of all accumulated sick leave and extended sick leave under Article 16.
22.6.4 For employees with five or more years of STRS service and two or more eligible children on the day of disability, benefits shall be payable for one year from the date of disability for both accident and illness provided that the employee is 69 years of age or younger on the date of disability. If the period of disability extends beyond one year, the employee shall receive disability allowance payments from STRS.
22.6.5 For all employees not included under Section 22.6.4, the benefit shall be payable for ten years from the date of disability for both accident and illness provided that the employee is 55 years of age or younger on the date of disability. If the employee is older than 55 years on the date of disability, the maximum benefit period shall be the same as that provided in the maximum benefit schedule set forth in the District’s income protection insurance policy.

18.9.1 The insurance shall provide a disability payment equal to 66 2/3% of the worker's "basic monthly earnings" on the date he/she was disabled to a maximum payment of $4,300 per month. "Basic monthly earnings" means 1/12th of the worker's annual contract salary.
18.9.2 The disability payment under the long-term disability insurance shall begin after all full-pay sick leave and extended sick leave under Article 10 has been used.
18.9.3 For workers with five years or more of STRS service and two or more eligible children on the date of disability, disability payments shall be payable for one year from the date of disability for both accident and illness provided that the worker is sixty-nine years of age or younger on the date of disability.
