MEDICATION POLICY. CONTRACTOR shall establish a written Medication Policy, which shall be reviewed and approved by ADMINISTRATOR. The policy shall include but not be limited to the securing, handling, and administration of Resident prescribed medication(s). Such policy shall address medications that are prescribed for substance and mental health disorders and medications disallowed by CONTRACTOR. Residents shall have medications during their stay with the program, and/or to have the ability to get refill(s).
MEDICATION POLICY. New Odyssey for Children will not dispense prescription medication without specific written authorization from the child’s parent and physician Non-prescription medication will not be dispensed without specific written authorization from the child’s physician and parent. Medications will only be administered at 11:00AM and 3:00 PM. • Medications must be in their original container clearly labeled with medicine name. • Medications must be clearly labeled with the child’s first and last name. • Prescription Medications must be clearly labeled with Prescription Number. • Medications must be clearly labeled with dosage amount.
MEDICATION POLICY. The district does not dispense Tylenol, Tums, cough drops, or tissues. If you want your child to have access to Tylenol and/or Tums, please send the items in a bottle marked with your child’s first and last name, parent’s first and last name, and teacher’s name. A written permission note with your child’s name, xxxxxx’s name, teacher’s name, date, dosage, and your signature must accompany the medication. Special medications or prescriptions for students will only be adminis- tered to students as follows:
MEDICATION POLICY. The Charter School shall develop and submit as part of the Start-Up Protocol a policy as to administration of medication to students. Once approved, the policy shall be attached to this Charter as Schedule 26.
MEDICATION POLICY. As required by law, a doctor’s written orders will be required if your child must take medica- tion when attending the center. This includes prescriptions.
MEDICATION POLICY. NOTE: If possible, the parent/legal guardian is advised to give medication to the student at home and on a schedule other than during school hours. Only oral, aerosol/inhalant medication in pre-measured dosages, topical ointment for diaper rash, and emergency medications shall be administered by school-based personnel, unless otherwise provided for in this policy. Xxxxxxxxx Xxxxxx Public School employees are not allowed to alter medication or administer any medication that exceeds the recommended dosages. Students are not permitted to have in their possession any medication (prescription or non-prescription) while under school supervision except when ordered by the physician for emergency medication (i.e., asthma inhaler, epinephrine, etc.) and after consultation between the parent/legal guardian, designated school staff, and the school nurse. Xxxxxxxxx Xxxxxx Schools, in compliance with the most current Medication Policy for Louisiana Public Schools adopted by the State Board of Elementary and Secondary Education, will provide for administration of medication to a student after the following procedures and responsibilities have been implemented:
MEDICATION POLICY. Xxxxx Center School Board Policy states that other than bandaging of minor cuts, bruises and xxxxx, NO medication, including aspirin, ointments, cold tablets, etc. shall be administered to students without written instructions on a form provided by the school and signed by the parent or guardian. Students that require prescription medications administered during the school day need to have a medication form on file signed by a physician before medications can be administered. Prescription medications need to be in a prescription bottle, with the prescription dosage clearly stated. Approved medications will be distributed at the appropriate time through the main office. Prescription medications given for less than ten days, do not require a physician signature. Students with appropriate written permission from the physician and a parent, may possess and use a metered dose inhaler or dry powder inhaler to alleviate asthmatic symptoms. Students with appropriate written permission from the physician and a parent, may possess and use an Epi-pen (epinephrine) to treat a severe allergic reaction. Xxxxx Center Public Schools – Helping students build success stories. Non-school sponsored student groups organized for religious, political, or philosophical reasons may meet during non-instructional hours. The application for permission may be obtained from the principal. The application must verify that students are initiating the activity, attendance is voluntary, and that the event will not interfere with school activities. School rules will still apply regarding behavior and equal opportunity to participate. Non-district sponsored organizations may not use the name of the school or school mascot.
MEDICATION POLICY. CONTRACTOR shall have a written policy and procedure for the 13 handling of all medications, including over the counter items. The policies and procedures shall include 14 at a minimum, logging, tracking, storing, disposing, and the distribution of all medications.
15 a. CONTRACTOR shall collect from all Participants, any and all prescription and over-
17 1) CONTRACTOR shall document in a medication log book, all medications, and 18 include, at a minimum, the name of the Participant, medication name, dosage, and quantity.
19 2) CONTRACTOR shall ensure that no medication is transferred between bottles.
20 3) CONTRACTOR shall ensure that no Participants have prescription medications or 21 over-the-counter medications in their possession and/or living area.
22 b. Following forty-eight (48) hours of Participant leaving the program, CONTRACTOR 23 shall destroy all medication Participant did not take with him/her. Destruction of said medication is to 24 be documented by CONTRACTOR in the medication log book and include, at a minimum, the name of 25 the Participant, medication name, dosage, quantity, and date of destruction.
26 c. CONTRACTOR’s staff must be trained on and familiarized with policies and 27 procedures regarding the handling of all medications.
MEDICATION POLICY. The staff do not administer any medications except “life-saving” medications, such as inhalers and Epi-Pens. Aftercare follows the medication policy of St. Xxxxxxxxx of Siena Catholic School as outlined in the Parent/Student Handbook. Parents of students who require “life-saving” medications must provide these medications for Aftercare staff, since the staff will not have access to the clinic where these medications are stored for use during the school day. Should a child have any adverse reactions to life-saving medications, parents will be called immediately. If the staff deems it necessary, 911 will be called. To ensure the safety of your child, all students must be escorted from Aftercare by an authorized adult to the car after being signed out. I request to enroll my child in the Aftercare Program for the 2022-2023 school year. MEDICATION POLICY. FSB only provides medication services to individuals who are enrolled in and consistently receive psychotherapy services through FSB. You must receive weekly or bi-weekly psychotherapy to receive medication services.
