Missed Sessions. Any missed sessions must be made up through watching the recordings and answering quesitons or with one of the primary instructors at $150/hour. If you know that you will miss an entire day due to a prior commitment, please contact Xxxxx Xxxxxxxx at PRAJNA Advanced Studies Program in advance of signing up for the program to discuss options. We will be recording the lectures incase of emergencies or other unpredicted circumstanes, but highly recommend attending in-person. Core concepts that are not understood or demonstrated by the student through homestudy, experiential learning or practice teaching may require additional mentoring by one of the lead instructors or assigned faculty and will be charged at $150/hour.
Missed Sessions. (i) If a client misses a session, or doesn’t show up for session, without providing 24 hours notice, then in most cases full payment is still required and a refund will not be offered.
(ii) You agree to give 24 hours to cancel or reschedule a session.
Missed Sessions. 7.1. Where an LMT tutor is unable to deliver a pre-planned session (e.g. through illness), LMT will endeavour to provide a suitable cover tutor for that lesson. If a school does not wish a cover tutor to attend or if LMT is unable to provide a cover tutor for the planned session, the cost of that lesson will be reimbursed (see 6.2).
7.2. Although not guaranteed, it may be possible for a LMT tutor to change the time or day of their lesson to accommodate a school closure or a pre-planned event. However, at least four weeks’ notice should be given, in writing to xxxx@xxxxxxxxxxxxxx.xxx and, if it is not possible, the cost of that lesson will be reimbursed (see 6.2). Notification given over the telephone or via tutors is not acceptable.
7.3. Where a school has to cancel a pre-arranged session without giving four weeks’ notice – including as a result of unexpected circumstances (e.g. adverse weather conditions where the school has closed) – the cost of that lesson will not be reimbursed.
7.4. Notice to cancel a Curriculum Extension project must also be made in writing (as per 7.2, above). Where notice is given more than eight weeks in advance of the commencement date of the project in question, there will be no charge due from the school. Where notice is given less than eight weeks but more than four weeks in advance of the commencement date, the school will be liable for 50% of the project cost. Where notice is given less than four weeks before the commencement date, the school will be liable for the full cost of the project.
7.5. A charge of £46.00 per hour will be made to any school wishing to book an additional Curriculum Extension project session i.e. they do not or are unable to attend a scheduled Singfest rehearsal, but would like a visit made to their school in lieu of this.
Missed Sessions. If a participant misses a session, their Group Program Badge is valid for a 4-hour Lift Only make-up visit on Mondays through Fridays, excluding holidays (December 23-27, 30, 31, 2019; January 1, January 20, 2020; February 17-18, 2020), until the end of season. There is no make-up, credit or refund for a missed lesson. If a participant purchased the rental option, they will be able to use the rentals on the make-up visit if prior arrangements are made with the Rental Shop. You can contact the Rental Shop at (860) 379-7669 ext. 250. ● A refund, less a $35 fee, will be given if a participant cancels out of Ski Club after November 15th. The request must be emailed to Farmington Continuing Education at xxxxxxxxxxxx@xxxxx.xxx. ● No refunds will be given if a participant is absent from school or unable to attend a session. ○ In the event of injury or illness that prevents a participant from completing the program, under the protection policy, the participant will be reimbursed the price of their program on a prorated basis. ○ Please notify Farmington Continuing Education at xxxxxxxxxxxx@xxxxx.xxx in this event. ○ The illness or injury must be confirmed by a letter from a licensed physician. Refunds will not be provided without this documentation.
Missed Sessions. If a scheduled appointment needs to be cancelled, please notify me more than 24 hours in advance. A charge equal to what your insurance would have paid for our regular appointment will be charged to you.
Missed Sessions. If you are unable to attend the line dancing session, it will be lost. Scheduled sessions will continue as planned unless otherwise advised by the YMCA (i.e., facility is closed for a holiday/event). It is the member’s responsibility to be present for scheduled sessions.
Missed Sessions. If you have a live chat arranged and you cannot attend, please send your counsellor a message as soon as you know you are not going to be able to attend the session.
Missed Sessions. If you need to cancel a session for a reason other than illness or emergency, I ask that you give at least 24-hours notice. If a session is missed or canceled without 24-hour notice, you are responsible for paying for a missed session fee of $75. If you are late to a session, you are responsible for the full session fee. Insurance cannot be used to cover missed session fees. I may not be immediately available by telephone or email. In the case of an emergency, always call 911 and/or proceed to your nearest emergency room. You are welcome to leave a voicemail and I will return your call as soon as I am able, typically within 24 hours. Voicemail messages and email are not appropriate for communicating emergencies. If I am unavailable for an extended period of time, I will have another therapist cover; I will let you know who that person is and how you can reach them. The laws and standards of our profession require that we keep treatment records. You and your child (if age 12 years or older) have the right to inspect and copy your records. Please see the Notice of Policies and Practices to Protect the Privacy of Your Health Information (provided to you) for more information about my professional records.
Missed Sessions. No refunds are given for missed sessions. Two (2) Make-up lessons are available in each 12 weeks block; the Centre must be notified via call or email at least one hour prior to the class that they are unable to attend their scheduled class. A makeup class must be redeemed within the same block that the scheduled class was missed and the Centre notified. If the makeup class is not scheduled during the same block in which the class was missed, the make-up lesson is forgone. Where a makeup cannot be offered due to class availability, no credit will be offered. Make-up classes can only be used for a class equivalent to the members enrolled class level; they cannot be used forother classes on the Timetable, Casual Classes or Centre Events. If your child is missing class due to medical reasons, the Centre must to be notified prior to their scheduled class. If the medical reason will impact their Gymnastics classes for up to 6 weeks a medical certificate is required, once a medical certificate is received we are able to hold their place in their current class and suspend their payments for a maximum of 6 weeks. Classes cannot be backdated prior to the date the Centre was notified. If the member is able to return before the date on their Medical Certificate a Medical Clearance is required before they can resume classes. Medical Certificates will only apply for a suspension of up to 6 weeks maximum. After this date, the booking will be cancelled if class is not resumed.
Missed Sessions. Swimmers are eligible for a set number of missed sessions per calendar year dependent on level (See Reverse). These can be used as either a rescheduled session (must email at least 24 hours in advance to book) or as a family Swim Pass which allows up to 5 people entry to the pool (max.2 adults). The Centre must be notified at least 30 minutes prior to the missed session. Notifications will be valid 31st December each year. Any unused will expire and cannot be extended. Cancellation inimum of 30 days’ notice is required to cease lessons. Notification is required in writing via the cancellation form. The form must be completed in the Centre and cannot be emailed or sent out to you. No emails will be accepted for cancellations, nor will cancellation requests be backdated to the date of an email sent regarding cancellation. No refund will be given for any monies paid in advance unless a medical certificate or proof of relocation is provided (needs to be more than 25kms from the Centre to qualify). Refund will be calculated from the date the Swim School is notified. I declare that my child is physically medically fit and capable to engage in the squads program at the Centre. I have and will inform in writing the Squad coaches at MABC Aquatic Centre of any condition or risk that may have an effect on my child’s/xxx’s ability to participate in the program prior to commencement. "I hereby give MABC Aquatic Centre as my permission to film my child/ren within the training session to provide feedback on technique for stroke development. I understand this footage will be used in house and for self reflection purposes.
