Payment Plans. Employees covered by the Samaritan Choice medical insurance plan who have outstanding balances that are payable to Samaritan Health Services for in network, covered, and authorized (if medically necessary) services will be provided payment plan offerings upon request from the employee. The request will be made to Patient Financial Services, and may be directed through the Hospital Patient Financial Counselor. Patient Financial Services will work with employees to identify the appropriate payment arrangement based on the employee financial needs/eligibility. Within 120 days from first patient statement, employees must contact Patient Financial Services and identify themselves as a SHS SEIU member and ask for a payment plan arrangement that does not exceed six percent (6%) of their household income. Such requests will be granted using the existing SHS payment options and funding programs. To be eligible for a payment plan, employees must comply with all requirements for establishing appropriate payment options/eligibility, including the completion of a financial assistance application with supporting documentation. Employees who comply with all terms of the payment plan(s) will not be subject to collections or wage garnishment.

To take advantage of a payment plan, I must sign up after I receive e-mail notification of the first e-bill of the semester. All monthly payments are processed automatically on the 20th of each month. The amounts I need to pay may change due to discrepancies between my student account and payment plan agreement amount resulting from changes to my bill, such as additional Colorado Opportunity Fund stipend for in-state undergrads, tuition waivers, financial aid, payments, adding/dropping classes etc.  A rebalancing of my payment plan agreement resulting in decreased future payment amounts will be completed automatically before the next payment withdrawal date.  A rebalancing of my payment plan agreement resulting in increased future payment amounts will require an action on the part of the Payment Plan owner. This will not happen automatically. o Any student account balance not covered by the scheduled payment plan payment amounts will be due immediately following the final plan automatic deduction date and will be subject to late fines and services charges.

Proof of the date of birth, acceptable to the Company, must be furnished for each person on whose life the payments are based.
Payment Plans. 6.1 The Price and the Extras & Deposits (collectively the "Total Payable Amount") shall be paid to the Developer strictly in accordance with the schedule of payment provided in the Application Kit (“Payment Schedule”). The Payment Schedule is based on tentative measurements of the Apartments. By applying for an Apartment in the prescribed Application Form of the Developer, the Allottees will be deemed to have authorised the Developer to receive payment in this manner and undertakes not to object to such adjustments made by the Developer. 6.3 The Applicants are required to indicate their preference of payment plan in the Application Form. Change of preferred payment plan shall be allowed by the Developer at its sole discretion and with or without such conditions as it may deem fit. 6.4 The Total Payable Amount in the above plans is exclusive of any taxes which may be leviable by any appropriate authorities. Taxes like Value Added Tax, Works contract tax, G S T, any other tax, both present and future, as may be applicable, shall be separately charged and recovered from the Allottees. All the applicable taxes shall be payable by the Allottees as and when demanded by the Developer.

Payment Plans. I have the option of signing up for a payment plan that spreads the cost of tuition and fees over a period of months (depending on when charges were assessed to me). The payment plan options require a bank account or credit/debit card to be set up for automatic withdrawal. There is a $35 fee for taking advantage of the payment plan. There is also a 2.75% service fee assessed to all credit/debit card payments. E‐check (bank account) transactions are not assessed the 2.75% fee. Funds will automatically be pulled on the 20th of the month, for each month of the plan. For example, in the spring semester:  1/4 of the charges will be automatically withdrawn on January 20th  1/4 of the charges will be automatically withdrawn on February 20th  1/4 of the charges will be automatically withdrawn on March 20th  1/4 of the charges will be automatically withdrawn on April 20th If the automatic withdrawal fails due to insufficient funds or invalid account information, a second attempt will be made approximately a week later. Notification will be sent in the case of a failed attempt, providing enough time for me to correct inaccurate information, or to change funding sources. If the second attempt fails, a $30 returned payment fee will be applied, and I will be removed from the payment plan and may be subject to additional service charges. Past due charges will be applied while you are actively making payments. The monthly payment amounts may change due to recalculations resulting from changes to my bill, such as adding or dropping classes, tuition waivers, financial aid, etc. If this occurs, I understand and agree to take action to rebalance my payment plan by logging in to my student portal via UCDAccess and following the on‐screen instructions. I agree to abide by the Terms and Conditions of the Payment Plan Agreement if I choose to enroll. Payment Plans. If you cannot pay your bill, we may offer you an instalment plan and/or ask for a security deposit instead of fitting a prepayment meter. While you still owe us money under your instalment plan, you may not be able to change suppliers. If you move, you will have to pay us the full amount you still owe under the instalment plan, unless you ask us to transfer the amount over to your new address and you continue to take your supply from us. If you fail to make any payment due under the instalment plan, the plan will automatically end and we may install a prepayment meter on the property. We may change your payment method upon 7 working days' notice if you have not paid your bill within 28 days of the due date, and/or have failed to comply with terms and conditions relating to a payment method. We will advise you of the proposed change, reason for and effect of the change and the price that you will pay in the future.
