Nature and Scope of Services. As a Volunteer, I will be contributing my time and effort to various events and programs of UVAR and performing a wide range of Services (collectively, the “Services”). The Services will be performed by me as assigned by an officer, director, employee or another volunteer of UVAR. In the course of performing the Services, I agree: • To read the Volunteer Handout and otherwise complete all required orientation, training, and paperwork relating to my volunteer position(s). To disclose any physical or psychological limitations to appropriate staff before participating in any activity. Since I may be interacting with animals, both healthy and sick, big and small, and may be lifting, carrying, moving, or otherwise engaging in physical labor, I will be respectful of my own limitations and will inform staff immediately of any such limitations. • To read and to obey all safety rules and regulations. In the interest of the safety of the animals, staff, and volunteers, I acknowledge that UVAR has the right to revoke volunteer privileges if these rules and regulations are not followed. • To treat all UVAR staff, volunteers, THE ANIMALS, property, tools, and equipment with respect and kindness. I will also return all UVAR property when my volunteer relationship ends. • That either I or UVAR can terminate this relationship at will.

Nature and Scope of Services. The particular nature and outcomes of the services herein contracted for are outlined in the Community Partner’s proposal to United Way. For this funding period, the Community Partner’s most current programs and outcome measurements are contained in the Community Partner’s proposal to United Way. The services contracted for, pursuant to this Agreement, shall be performed in a manner acceptable to United Way, and in strict compliance with all applicable federal, state, and local laws, and regulations which are in existence at the commencement of this Agreement and which may be adopted or amended during the term of this Agreement. The Community Partner must notify United Way within 24 hours of 501(c)(3) status revocation, which will lead to a suspension and reevaluation of future activities, and may lead to Termination for Cause pursuant to Section 1 of this Agreement. During this funding period, the outcomes outlined in the proposal may be modified and altered by the Community Partner; however United Way must be notified, in writing, of all such modifications and alterations. If there are proposed adjustments to program activities and procedures, there shall be joint consideration of how such change might affect the current and future budgets of the Community Partner and the total needs and services of the community.

Since I may be interacting with dogs and may be lifting, carrying, moving or otherwise engaging in physical labor, I will be respectful of my own limitations and will inform SDR immediately of such limitations. In the interest of the safety of the dogs, the public, and other volunteers I acknowledge that SDR has the right to revoke volunteer privileges of these rules and regulations are not followed. TO TREAT ALL SDR OFFICERS, DIRECTORS, VOLUNTEERS, DOGS AND MEMBERS OF THE PUBLIC WITH WHOM I INTERACT WITH KINDNESS AND RESPECT. I ACKNOWLEDGE THAT I AM VOLUNTEERING FOR THE BEST INTERESTS OF THE DOGS AND NOT FOR ANY PERSONAL GAIN OR TO FURTHER ANY PERSONAL AGENDA.

I understand that, in the event of poor or unsatisfactory performance or poor attendance, The Oasis has the right to dismiss me, and that either I or The Oasis can terminate this relationship at will (at any time, for any reason or for no reason at all). • To disclose any physical or psychological limitations to appropriate staff before participating in any activity. Nature and Scope of Services. (A) During the Term of this Agreement, Consultant will provide Golf One services with respect to administration and maintenance of certain corporate records, advise Golf One regarding proper and timely corporate filings as required under state and federal law, review certain corporate contracts and documents and to develop group business opportunities for Golf One during the Term. Consultant shall have the status of independent contractor. In the fulfillment of Consultant's responsibilities hereunder, Consultant shall devote such time and attention to, and use Consultant's best efforts to, manage certain corporate records and advance the business and welfare of Golf One as Consultant shall deem reasonably necessary.

ANY (A) ADVICE, ASSISTANCE, RECOMMENDATION OR DIRECTION PROVIDED BY CONSULTANT WITH RESPECT TO THE DEVELOPMENT, DESIGN, CONSTRUCTION, EQUIPPING, FURNISHING, OR DECORATION OF EACH PROJECT; (B) REVIEW AND APPROVAL OF ANY CONCEPTUAL, PRELIMINARY OR FINAL PLANS AND SPECIFICATIONS FOR EACH PROJECT; (C) PERIODIC REVIEW OR INSPECTION OF THE CONSTRUCTION OF EACH PROJECT; AND, (D) OTHER SERVICES PROVIDED TO OWNER BY CONSULTANT AND/OR ITS AFFILIATES UNDER THIS AGREEMENT: ARE PERFORMED SOLELY FOR THE PURPOSE OF PERFORMING THE SERVICES, INCLUDING (1) ASSISTING OWNER IN THE DEVELOPMENT AND CONSTRUCTION OF EACH PROJECT, (2) DETERMINING THAT THE PLANS AND SPECIFICATIONS INCORPORATE THE STANDARDS AND GUIDELINES SET FORTH IN THE DESIGN SPECIFICATIONS AND THE MANAGEMENT STANDARD; AND, (3) REVIEWING THE PROJECT FROM A FUNCTIONAL AND AESTHETIC POINT OF VIEW; AND DO NOT CONSTITUTE ANY REPRESENTATION, WARRANTY OR GUARANTY OF ANY KIND WHATSOEVER, INCLUDING THAT (1) THERE ARE NO ERRORS IN THE PLANS AND SPECIFICATIONS; (2) THERE ARE NO DEFECTS IN THE DESIGN OR CONSTRUCTION OF ANY PROJECT OR INSTALLATION OF ANY BUILDING SYSTEMS OR FF&E THEREIN; OR, (3) THE PLANS, SPECIFICATIONS, CONSTRUCTION AND INSTALLATION WORK WILL COMPLY WITH ALL APPLICABLE LAWS, INCLUDING THE AMERICANS WITH DISABILITIES ACT OR SIMILAR LAWS OR REGULATIONS GOVERNING PUBLIC ACCOMMODATIONS FOR INDIVIDUALS WITH DISABILITIES.
Nature and Scope of Services. Agency must provide services or functions within one or more of United Way’s four Focus Areas – Education, Income, Health, or Safety Net services. The particular nature and outcomes herein contracted for, are outlined in Exhibit A. During this funding period, the outcomes outlined in Exhibit A may be modified and altered, however UWRC must be notified in writing of all such modifications and alterations and approve all changes. If there are proposed adjustments to program activities and procedures, there shall be joint consideration of how such change might affect the program’s purpose and outcomes.
