Needs Improvement Evaluation. 1. An employee who receives an overall needs improvement evaluation shall be provided a Performance Improvement Notice within ten (10) days of such receipt that contains at least the following information: a. A description of the performance that needs to be improved; b. The performance improvement desired; c. Assistance to be provided the employee including supervisory feedback, training, etc.; d. Length of time within which to achieve the improvement; and e. Possible consequences for failure to improve performance. 2. An employee shall be provided a total of at least sixty (60) days within which to improve performance to a satisfactory level. The employee’s performance shall be reevaluated within ten days of the conclusion of the performance period. 3. Annual salary increases shall not be withheld based on a needs improvement evaluation.
Needs Improvement Evaluation. If the overall rating is “needs improvement,” the evaluation team shall forward a remediation plan to the appropriate ▇▇▇▇/Area Manager and Vice President for review. After the Vice President, ▇▇▇▇/Area Manager and evaluation team finalize the remediation plan, the evaluation team shall present the plan to the evaluatee. The remediation plan will include a timeline for completion and follow-up evaluation. a. If the evaluatee receives a second overall rating of “needs improvement,” both of the evaluation reports and remediation plans will be forwarded to the appropriate Vice President for administrative review. The evaluatee will not be eligible for a summer session assignment, or overload assignment in the ensuing Fall Semester. b. If the evaluatee receives an overall rating of “needs improvement” in the Fall Semester, they will not be eligible for an overload assignment in the Spring Semester. If the evaluatee receives an overall rating of “needs improvement” in the Spring semester, they will not be eligible for a summer session assignment, or an overload assignment in the Fall semester. c. If the evaluatee successfully completes each requirement of the remediation plan, and receives an overall “satisfactory” rating in the follow-up evaluation report they will be eligible for overload and/or summer session assignment(s).

A scheduled increase will, however, be withheld if an employee’s performance does not improve to an overall Meets Expectations level on the next following annual evaluation.

Needs Improvement Evaluation a. During the observation process if an evaluator detects area(s) of deficiency he/she shall provide the employee mutually agreed upon assistance before the Evaluation Summary takes place. b. If the Evaluation Summary received indicates Needs Improvement the evaluatee will participate in the evaluation process the following year. The evaluator will provide mutually agreed upon assistance during the following year to remedy any deficiencies noted in the Evaluation Summary.

  • School Improvement The parties do hereby mutually agree that the school improvement process currently in effect will continue. Any plan developed by the committees shall not be in conflict with the master agreement or board policy.

  • Quality Improvement The Parties must develop QI activities specifically for the oversight of the requirements of this MOU, including, without limitation, any applicable performance measures and QI initiatives, including those to prevent duplication of services, as well as reports that track referrals, Member engagement, and service utilization. Such QI activities must include processes to monitor the extent to which Members are able to access mental health services across SMHS and NSMHS, and Covered Service utilization. The Parties must document these QI activities in policies and procedures.
