Notification by Employee. When an employee is unable to report to work he shall notify his immediate supervisor or other designated person at least one (1) hour before the time he is scheduled to report to work on each day of absence, unless the employee has made other reporting arrangements with his immediate supervisor.

An employee who is unable to work shall notify his/her immediate supervisor or the communications specialist as soon as possible, but not later than two (2) hours before his/her scheduled work time on the first day of absence and each day thereafter, unless circumstances prevent such notification.

The administration may require a signed physician's statement attesting to such illness or injury or require that the employee provide a written statement giving the name, address, and phone number of his physician and the dates he consulted with him.

If the Plan is not notified by a Participant that such Participant’s limitation of Salary Deferral Contributions (under Code section 402(g)) as described in paragraphs (a) and (b) of this Section 3.06 has been exceeded, the Plan shall assume that such limitation has not been exceeded by such Participant’s Salary Deferral Contributions to the Plan.

An employee shall report the intention to be absent from duty to the Superintendent or designee by 7:00 A.M. on the day of the absence, provided that if possible, notification should be given on the previous day or earlier. If an employee expects to return to an assignment, the employee should notify the Superintendent or designee of such intention by 3:00 P.M. on the previous day in order to release the substitute.

When an employee is unable to report to work, he shall notify the Fire Chief or his designated representative, one (1) hour before the time he is scheduled to report to work on each day of absence, unless emergency conditions make it impossible, or other arrangements have been made in advance with the supervisor.

When an employee is unable to report to work, he/she will notify his/her Immediate Supervisor and/or other designated person, no longer than ten (10) hours or less than one (1) hour prior to the time he/she is scheduled to work on each day of absence. Failure to so inform on each occasion, or at agreed intervals in the case of extended illness, will result in a loss of that day's pay. Continued abuse of this Section will give cause for dismissal from service with the City.

When an employee is unable to report to work, the employee shall telephone and speak to his/her immediate supervisor or other designated person no later than fifteen (15) minutes after the beginning of the employee’s scheduled work day on each day of absence, unless the employee has made other reporting arrangements with his/her immediate supervisor. Employees in divisions/units that require different notification procedures will be given written instructions.
