Notification of Xxxx Leave. In the event an employee must take sick leave, the employee must notify or leave a message with the employee’s Department Head at least one hour before the employee’s scheduled reporting time. Failure to give this notice will render the employee ineligible to use sick leave credits for the absence.

Any employee who is unable to report for work due to illness or is delayed, is responsible for insuring that his/her immediate supervisor is notified as far in advance of the start of the employees scheduled shift or within one hour prior to or after the starting time, or in an emergency situation as soon as is practical. Supervisors shall post or distribute the names of contact persons, alternate contacts and phone numbers.

An employee shall ensure that adequate notice is given to their immediate Supervisor as early as possible before the employee is due to start work on the day that sick leave is to be taken; or if that is not practicable, as early as possible after that time. Where applicable the employee shall state the projected date of return to work and shall notify Barnardos if this date changes.
Notification of Xxxx Leave. The employee is responsible for notifying the Department each time sick leave is to be taken. The employee shall notify the Department a minimum of two (2) hours before the start of their scheduled shift.

These procedures must be followed to receive paid sick leave. Employees may use sick pay to supplement disability pay up to their regular weekly pay. In the event that an employee runs out of sick leave due to a medical hardship the Town shall allow other employees to donate up to five (5) days of sick, and / or any amount of personal and / or vacation time to said employee on a cost per hour, pro-rated basis based on the hourly rate of the donor.
Notification of Xxxx Leave. Any employee who cannot report to duty because of illness or disability shall immediately notify the Department Head, or designee, as soon as possible before the employee's scheduled starting time and shall state the nature of the illness or disability. If an employee cannot notify the Department Head, or designee, because of the serious nature of the illness or disability, a responsible person may make the report on behalf of the employee. For any false representation made by an employee in connection with a claim for sick leave benefits, the employee shall be subject to loss of the sick pay benefits falsely obtained or may be subject to dismissal.
