Evaluation and Reporting. For each measure included in its Academic Performance Framework, schools will set targets for each year of their contract. The Commission will publicly report these targets, as well as each school’s performance on the measures, to the extent possible while protecting the privacy and confidentiality of students’ data. The Commission will also report whether the school met its annual target, exceeded the target, or did not meet the target. If the School does not achieve results within the set target range, the School is subject to the Intervention Protocol, provided in Exhibit D of this Charter Contract.
Evaluation and Reporting. Placement Site will submit required reports on each Student’s performance and will provide an evaluation to University on forms provided by University. Placement Site will notify University of any significant situation or problem that may threaten the successful completion of the Fieldwork Program by the Student.
Evaluation and Reporting a. The Contractor shall promptly provide Project reports with payment requests at least every three months, and upon request by the Commission. Project reports are subject to the Coastal Commission Executive Director’s review and approval. The Project report shall include a description of work tasks and deliverables completed to date, and a description of completed benchmarks, or progress toward completing benchmarks. In any event Contractor shall provide the Commission a report showing total final Project expenditures with the final Request for Funds and required closing documents. Contractor shall submit all documentation for Project completion, as applicable, and final reimbursement by the end of the contract period, April 30, 2016.
b. Final payment is contingent upon Commission verification that Project is consistent with the Scope of Work as described in Exhibit A- Attachment I, together with any Commission approved amendments.
c. Contractor must report to the Commission in the Project Budget all sources of other funds for the Project.
Evaluation and Reporting a) After an investigation has been completed and an evaluation has been prepared, the Chief of the Khartoum Office (as overall Mission Coordinator) will issue an incident report. The report will include both an analysis of the incident and, if a serious violation has taken place, recommended steps for the Parties to take to avoid similar incidents in the future.
b) The Chief of the Khartoum Office will provide the incident report to the Government and to the SPLM for comment. After the set period for comment (one week) has passed, the Mission will provide the final report including the comments or explanation of the Parties to the Government, the SPLM, and the Funding Partners. The report will also be made public.
Evaluation and Reporting. THINK TOGETHER agrees to supply the LEA with all reporting data explicitly required via written notification to the LEA by the CDE or U.S. Department of Education in advance of any deadlines. The LEA agrees to submit all reports required by the CDE or U.S. Department of Education in a timely manner in advance of deadlines and provide proof of submission to THINK TOGETHER. If the LEA prefers to have THINK TOGETHER submit reports directly to the CDE on the LEA’s behalf, the LEA shall provide THINK TOGETHER access to its CDE “ASSIST” account. The LEA will provide THINK TOGETHER with any pertinent grant-related communications within five working days of receipt from the CDE. THINK TOGETHER will comply with all document requests from the CDE in accordance Federal Program Monitoring requirements. Additionally, THINK TOGETHER will: • Provide monthly attendance and snack reports to the LEA five working days in advance of the deadline. • Provide quarterly expenditure reports to the LEA five working days in advance of the deadline. • Provide the Annual Outcomes Based Data for Evaluation report to the LEA ten working days prior to the deadline.
Evaluation and Reporting. 4.1 Reporting and program evaluation elements of this partnership are intended to demonstrate the value of the partnership and identify opportunities for system improvement. Evaluation and reporting activities will be intentionally constrained to ensure that undue burden is not created for staff responsible for this coordination agreement.
4.2 Upon commencement of the agreement, the Cities and Department of Housing Services staff, in partnership with other city liaisons, will work together to develop a reasonable reporting template to define an annual narrative and program outcomes report. This template can be amended annually as the parties seek continued process improvements through learned experience.
4.3 Annual reporting and evaluation process:
a. The Cities will each provide a narrative report describing the coordination activities that were achieved through this agreement by September 30 for the previous fiscal year (e.g., a narrative report for FY2024-25 will be due by Sept 30, 2025).
b. The Department of Housing Services will provide each of the Cities with annual outcomes data from the Homeless Management Information System (HMIS) to describe impact of services provided in the Cities by September 30 for the previous fiscal year (e.g., outcomes data for FY2024-25 will be due by Sept 30, 2025).
c. The parties will co-present these annual reports and findings to the Washington County Homeless Services advisory body and may also present to each of the Cities’ City Council, and the Washington County Board of County Commissioners, or other stakeholders as deemed mutually appropriate by the parties.
d. The parties will convene at least annually to evaluate program outcomes and make recommendations for system improvements in April-May of each year, in advance of the County budget and work plan cycle.
e. The parties will convene at least annually to review outcomes of this Homeless Services System City Liaison program to make recommended changes and improvements to this scope of work and overall contractual partnership.
f. The Cities may request a report of program outcomes or provider evaluation
Evaluation and Reporting. The Parties will jointly review the results and effectiveness of this MOU at the end of each calendar year. The MOU may be amended at any time by the written, mutual consent of the Parties. The approved amendment(s) will immediately become a part of this MOU.
Evaluation and Reporting. 1. Grantee shall collect and submit to HHSC or its designated evaluation contractor and/or SAMHSA the NOMS measures by required timeframes.
a. PIPBHC specific health outcomes must be collected at baseline, at six-month intervals post baseline, and at discharge.
2. Grantees will collect and report data needed for the SAMHSA Infrastructure, Prevention, and Promotion (IPP) performance measures as listed in the PIPBHC grant application.
3. In addition to PIPBHC measures, grantees will also report on other outcome measures and policies that have been incorporated into operating procedures.
4. Grantee will host internal team coordination meetings monthly to consistently consider issues or barriers to the provision of high-quality services, determine and implement solutions, and monitor results. Formal reviews of data will be conducted on a quarterly basis and reported to HHSC.
5. Grantee shall collaborate with HHSC and/or a third party contractor to measure outcomes of local efforts and contribute to project reports.
6. Grantee shall submit quarterly progress/status reports to the HHSC Project Director or designee to include a summary of activities related to and the progress made toward the completion of the deliverables set forth in this contract, as well as the status, barriers, and recommendations related to the accomplishment of these deliverables. A copy of any supporting materials for assessment activities will also be submitted.
7. Grantee shall use electronic health record (EHR) population management tools in order to support continuous quality improvement processes and must regularly generate reports by specific conditions to use for quality improvement, reduction of disparities, research, or outreach.
Evaluation and Reporting. The District agrees to submit all reports required by the CDE or U.S. Department of Education in a timely manner and in advance of deadlines. CONTRACTOR agrees to supply the District with any and all reporting information explicitly required via written notification to the District by the CDE or U.S. Department of Education in a timely manner and in advance of any deadlines. The District will provide CONTRACTOR with complete copies of all attendance, expenditure, and evaluation reports and pertinent grant-related communication within five working days of submission to CDE. CONTRACTOR will: • Provide monthly attendance and snack reports to the LEA five working days in advance of the deadline. • Provide quarterly expenditure reports to the LEA five working days in advance of the deadline. • Provide the Annual Performance Report to the LEA ten working days prior to the deadline.
Evaluation and Reporting. 11.1 Evaluation of progress in achieving the agreed outcomes and assessment of performance against any key performance indicators (KPIs) set by the EMHSCA committee, are to be undertaken annually by, or under the direction of the EMHSCA committee.
11.2 The EMHSCA project officer shall submit an annual written evaluation report to the EMHSCA committee on the achievement of deliverables/actions articulated in the Strategic Plan.
