Reporting of Sickness. An Employee who is absent from duty for reasons which entitle him/her to sick leave shall notify his/her Commanding Officer, or, if his/her Commanding Officer cannot be notified, his/her District Chief, prior to his/her usual reporting time, if physically able to do so. If an Employee knows he/she is going to be absent for more than one (1) day, he/she shall notify his/her Assistant Chief of such specified days. An Employee who continues to be physically unable to perform his/her normal duties shall be required to present each thirty (30) calendar days a physician's statement to his/her Assistant Chief confirming his/her continued inability to perform his/her normal duties. The Assistant Chief may at any time request a medical opinion from the City Physician regarding the Employee's ability to continue or return to work.

Reporting of Sickness. In the case of absence due to sickness or accident, the matter must be reported as soon as possible to the designated Employer representative in the worksite.
Reporting of Sickness. An employee who is unable to work because of sickness, accident or work injury shall report this as soon as possible to the employer or the person nominated by the employer. If there are valid reasons why a report cannot be made, the report shall be made as soon as this obstacle is removed. The employee shall also inform the employer as to when he or she expects to be able to return to work. The same applies to employees who have to stay away from work on account of risk of contagion. The employee is not entitled to sick pay for the time preceding the submission of said report.

  • Reporting of Sick Leave The employee will call in sick or returning in accordance with Article 14.05 to the designated employer for purposes of payment. For purposes of notification of absence, the employee will notify the appropriate home in accordance with Article 14.05 of their absence that shift.

  • Granting of Sick Leave An employee shall be granted sick leave with pay when he or she is unable to perform his or her duties because of illness or injury provided that:

  • Posting of Shift Schedules (a) Shift and standby schedules shall be posted at least four (4) weeks in advance of the schedule to be worked and the schedule shall be for a minimum of two (2) weeks. The Employer shall make every reasonable effort not to change shifts. If the Employer changes the shift schedule within forty-eight (48) hours of the shift, the Employee(s) affected shall be entitled to overtime compensation for that shift.   • Verification of Sick Leave 1. An employee who shall be absent on sick leave for three (3) or more consecutive working days shall be required to submit acceptable medical evidence substantiating the illness.

  • Reporting of Claims Consultant shall report to the City, in addition to Consultant’s insurer, any and all insurance claims submitted by Consultant in connection with the Services under this Agreement.

  • Posting of Schedules The Employer shall post the weekly work schedule for all employees not later than Monday 6:00 p.m. and twenty-two (22) days in advance.
