Service Guidelines. Medical Practice and its Users shall use the Software and any services provided by Modernizing Medicine solely for Medical Practice’s internal business purposes as contemplated by this Agreement and shall not use the Software or any services provided by Modernizing Medicine to: (i) send spam or any other form of duplicative or unsolicited communications; (ii) violate any law, rule or regulation; (iii) transmit through or post on the Software or Modernizing Medicine’s website(s) unlawful, immoral, libelous, tortious, infringing, defamatory, threatening, vulgar, or obscene material or material harmful to minors; (iv) transmit material containing software viruses or other harmful or deleterious computer code, files, scripts, agents, or programs; (v) interfere with or disrupt the integrity or performance of the Software or the data contained therein; (vi) attempt to gain unauthorized access to the Software, EMA Services, Additional Services, PQRS Services, computer systems or networks used to host or provide access to the Software; or (vii) harass or interfere with another user’s use and enjoyment of the Software or the services provided by Modernizing Medicine. Modernizing Medicine may, without liability or notice to Medical Practice, remove or delete any material stored in the Software that Modernizing Medicine determines, in its sole discretion, violates any of the guidelines set forth in this Agreement. In addition to any other remedies Modernizing Medicine may have, Modernizing Medicine reserves the right to terminate each of the Transaction Documents or terminate any or all of the logins provided to Medical Practice or any User immediately and without notice, if Modernizing Medicine becomes aware or determines that Medical Practice or any User is violating any of the foregoing guidelines. Medical Practice shall be responsible for verifying the accuracy of results produced using the Software and for proper use of any forms provided by Modernizing Medicine. Medical Practice shall be responsible for following proper backup procedures to protect against loss or error resulting from use of any or all of the Software.

Service Guidelines. Faculty who demonstrate documented service at levels commensurate with their rank shall be assigned 2 service credit hours for their annual workload assignment. Faculty who demonstrate documented service at levels below what is commensurate with their rank shall be assigned 1 service credit hours for their annual workload assignment. Faculty who do not demonstrate documented service shall be assigned 0 service credit hours for their annual workload assignment. Faculty who demonstrate documented service that exceeds levels commensurate with their rank may be assigned more than 2 service credit hours for their annual workload assignment, subject to the general guidelines delineated in 13.1.F

Service Guidelines. 1. This is a driveway snow removal service only; we do not currently offer shoveling of any kind. 2. This service is designed to clear the bulk of the snow from your driveway quickly and efficiently, blowing the snow into the front and/or side yard. 3. Our tractors will do most of the work for you, but not all. Those signing up for this service understand that they still may need to do some shoveling if they desire a very clean, manicured look. You may still desire to shovel in front of the garage door(s) and around parked cars in the driveway. Customer(s) Initials 1 4. How clean our snow blowers can scrape a driveway depends on many factors, including but not limited to driveway shape, slope, crown, grade, condition, type and timing of snow, sun exposure, air temp, pavement temp and humidity. Because none of these factors are within our control, we cannot guarantee that every driveway will look exactly the same at the end of each service. Some driveways scrape clean to the pavement every time, while others maintain a small coating of snow. 5. Our operators will do the best they can to get as close to the garage as is safe (usually about two feet). 6. We will clear Bump-Outs to the best of our ability. Due to the size of our tractors and their inability to execute sharp turns, there may be snow remaining in the corners. 7. Tuck-Behind Garages can be challenging for our tractors to navigate. Depending on your property, we may only be able to clear a path from the garage to the street. Sides or corners near the house may have snow left in them that we are unable to get. 8. Different tractor operators have different levels of comfort and ability when getting close to garages, landscaping features, etc. Within a snow removal season, we strive to have the same operator running the same route except in the case of illness or vacation, but there may be changes in operators from season to season. 9. While we do our best to clear in front of mailboxes as a courtesy to our customers, we are not responsible for mailbox clearing and will not do service calls for mailboxes that have been plowed in.
Service Guidelines. 2.1 The Lessor shall design and survey the installment location of the network system and WiFi Devices and all the accessories in order to provide the WiFi Service within the agreed location or area in accordance with engineering principles supporting the provision of WiFi service, as shown in Picture 2 and supporting the business alliance cooperating in providing the wireless internet network service with the details provided in the Appendix (5) to present to the Lessee and/or CAT Telecom PLC, to confirm the plans and the WiFi Access Point equipment installation list in each location or area prior to the installation. 2.2 Coordinate with location’s owner of each location or area in which the Lessee and/or CAT Telecom PLC has obtained permission from the location’s owner to enter the premises. The CAT Telecom PLC shall issue a letter or document as a proof to the Lessor to be used as reference for the installation and provision of network system and WiFi service equipment according to the provisions of the Agreement. 2.3 The Lessor shall supply and install all the relevant network system and WiFi Devices within the agreed location or area in each location according to the plans and installation points confirmed by the Lessee and/or CAT Telecom PLC and after the completion of the installation in each location, the Lessor shall submit the installation point plans presenting the actual structure of the location in electronic file format to store as Lessee’s and/or CAT Telecom PLC’s data base and as a reference for the collection of service fee provided in each location. 2.4 The Lessor shall maintain the network system and WiFi Devices and all the accessories within each location or area in a safe and usual functional condition throughout the conditions of the Google Station WiFi Service of the Lessee and/or CAT Telecom PLC. 2.5 The network system and WiFi Devices and all the accessories, which were supplied and installed by the Lessor, shall be able to connect with the telecommunication system and work with the WiFi Controller system of CAT Telecom PLC effectively to support the WiFi service according to the agreed service provisions between CAT Telecom PLC and the business alliance in cooperation of providing the wireless internet network service or WiFi Service. Service Guidelines. A. Contractor shall provide WA on a first-come, first-serve basis to households that income qualify and meet all LIHWAP eligibility requirements. B. In the event CSD issues a CSD Program Notice modifying eligibility requirements to provide financial assistance to eligible households to offset current bills, Contractor shall prioritize eligible households with past due water service bills, particularly those households that are at risk of being disconnected before providing benefits to eligible households with current bills. C. Contractor has discretion on the timeframe for accepting applications from customers based on factors such as when water systems in Contractor’s service area are enrolled in the Direct Payment Program or when Contractor has established operational readiness to being accepting application. D. Households that receive WA are not subject to the LIHEAP Local Agency Plan and Priority Plan points for purposes of receiving benefits under this Contract, per the Eligibility and Verification Guide and LIHWAP Special Addendum. E. Equitable Treatment Contractor shall ensure that low-income owners and renters that receive water services from an eligible service provider receive equitable treatment under this program. F. See Section 10.2 B, “Eligibility to Receive Federally Funded Public Benefits” concerning Federal restrictions on receipt of benefits.
